目的：探讨屈螺酮炔雌醇片（优思明）联合二甲双胍预处理后使用来曲唑治疗多囊卵巢综合征（ Polycystic ovary syndrome PCOS ）的临床效果。方法选择2013年1月至2014年1月首都医科大学附属北京妇产医院收治的104例PCOS患者的临床资料，其中52例给予优思明预处理后用来曲唑治疗，纳入对照组，52例给予优思明联合二甲双胍预处理后用来曲唑治疗，纳入观察组，比较两组治疗效果。结果两组预处理后促黄体生成素（luteinizing hormone，LH）、LH ／促卵泡刺激素（follicle－stimulating hormone，FSH）、睾酮（testosterone，T）显著降低（P＜0.05），观察组下降较对照组更为显著（P＜0.05）；观察组患者治疗后空腹胰岛素（fasting insulin，FIN）和胰岛素抵抗指数（insulin resistance index，HOMA－IR）明显改善（P＜0.05）；预处理后来曲唑治疗4个月，观察组的排卵率、妊娠率分别为88.4％、50.0％，高于对照组的71.4％、30.8％（ P＜0.05）。结论优思明联合二甲双胍预处理后来曲唑治疗PCOS临床效果满意，能够显著改善患者的激素水平和胰岛素抵抗，并提高排卵率及妊娠率。

목적：탐토굴라동결자순편（우사명）연합이갑쌍고예처리후사용래곡서치료다낭란소종합정（ Polycystic ovary syndrome PCOS ）적림상효과。방법선택2013년1월지2014년1월수도의과대학부속북경부산의원수치적104례PCOS환자적림상자료，기중52례급여우사명예처리후용래곡서치료，납입대조조，52례급여우사명연합이갑쌍고예처리후용래곡서치료，납입관찰조，비교량조치료효과。결과량조예처리후촉황체생성소（luteinizing hormone，LH）、LH ／촉란포자격소（follicle－stimulating hormone，FSH）、고동（testosterone，T）현저강저（P＜0.05），관찰조하강교대조조경위현저（P＜0.05）；관찰조환자치료후공복이도소（fasting insulin，FIN）화이도소저항지수（insulin resistance index，HOMA－IR）명현개선（P＜0.05）；예처리후래곡서치료4개월，관찰조적배란솔、임신솔분별위88.4％、50.0％，고우대조조적71.4％、30.8％（ P＜0.05）。결론우사명연합이갑쌍고예처리후래곡서치료PCOS림상효과만의，능구현저개선환자적격소수평화이도소저항，병제고배란솔급임신솔。
Objective To investigate the clinical efficacy of letrozole treatment after pretreatment of drospirenone and ethinylestradiol tablets ( Yasmin) combined with metformin in polycystic ovary syndromes .Methods Data of 104 cases with PCOS in Beijing Maternity Hospital Affiliated to Capital Medical University from Jan 2013 to Jan 2014 were selected , 52 cases treated with letrozole after pretreatment of Yasmin were in the control group , 52 cases treated with letrozole after pretreatment of Yasmin combined with metformin were in the observation group , the clinical efficacy of two groups were compared .Results The luteinizing hormone ( LH) , luteinizing hormone/follicle -stimulating hormone ( LH/FSH ) , testosterone ( T ) of two groups after pretreatment were significantly declined (P<0.05), the observation group declined more remarkable (P<0.05).The fasting insulin (FIN) and insulin resistance index (HOMA-IR) of observation group after treatment were significantly improved (P<0.05).With 4 months of letrozole treatment after pretreatment, the ovulation rate, pregnancy rate of observation group were 88.4%, 50.0%, which were significantly higher than the control group of 71.4%, 30.8%(P<0.05).Conclusion The pretreatment of Yasmin combined with metformin in polycystic ovary syndrome has a satisfactory clinical efficacy , which can significantly improve the hormone levels , insulin resistance , ovulation rate and pregnancy rate .
