目的:分析凝血检验危急值用于重症新生儿病情预测价值。方法回顾性分析2008年1月~2013年1月我院收治的478例重症新生儿凝血检验结果及凝血功能。结果478例重症新生儿中,凝血检验危急值49例,占10.3%。Fg低值28例:肺炎12例,呼吸窘迫综合征7例,脓毒血症5例,胎粪吸入综合征2例,高胆红素血症2例;APTT高值8例:脓毒血症6例,窒息2例;PT高值7例:呼吸窘迫综合征5例,维生素K缺乏症2例;Fg低值合并APTT高值2例:窒息;Fg低值合并PT高值3例:脓毒血症;APTT高值合并PT高值1例:呼吸衰竭。结论凝血检验危急值用于重症新生儿病情预测结果可靠,可以及时跟踪病情变化,降低死亡率,因此,值得临床推广和应用。

목적:분석응혈검험위급치용우중증신생인병정예측개치。방법회고성분석2008년1월~2013년1월아원수치적478례중증신생인응혈검험결과급응혈공능。결과478례중증신생인중,응혈검험위급치49례,점10.3%。Fg저치28례:폐염12례,호흡군박종합정7례,농독혈증5례,태분흡입종합정2례,고담홍소혈증2례;APTT고치8례:농독혈증6례,질식2례;PT고치7례:호흡군박종합정5례,유생소K결핍증2례;Fg저치합병APTT고치2례:질식;Fg저치합병PT고치3례:농독혈증;APTT고치합병PT고치1례:호흡쇠갈。결론응혈검험위급치용우중증신생인병정예측결과가고,가이급시근종병정변화,강저사망솔,인차,치득림상추엄화응용。
Objective To analyze the predictive value of coagulation test critical values for severe neonatal. Methods The coagulation test results and coagulation function of 478 cases of severe neonatal from January 2008 to January 2013 in our hospital were retrospectively analyzed. Results Among 478 cases of severe neonatal, coagulation test critical values of 49 cases, accounted for 10.3%. The low Fg value in 28 cases:12 cases of pneumonia, 7 cases of respiratory distress syndrome, 5 cases of sepsis, 2 cases of meconium aspiration syndrome, 2 cases of hyper bilirubinemia; The high APTT value in 8 cases: 6 cases of sepsis, 2 cases of asphyxia; The high PT values in 7 cases:5 cases of respiratory distress syndrome, 2 cases of vitamin K deficiency;The low Fg value and the high APTT value in 2 cases:asphyxia;The low Fg value with high PT value in 3 cases:sepsis; The high APTT value with high PT value in 1 case:respiratory failure. Conclusion Coagulation test critical values for predicting severe neonatal are reliable, and it can timely track the changes of disease condition, reduce the mortality rate, so,it is worthy of clinical promotion and application.
