目的：探讨股骨头坏死骨盆倾斜角度对全髋关节置换术后髋关节功能的影响。方法：50例患侧盆下型骨盆倾斜股骨头坏死患者均行全髋关节置换术，手术前术后行X线检查，测定骨盆倾斜角、髋臼外展角和前倾角，末次随访时评价髋关节功能。结果：骨盆倾斜患者术后髋臼外展角平均为III型（37°）<II型（40°）<I型(44°，P＜0.05），末次随访髋关节功能评分I型、II型明显高于III型（P＜0.05）。结论：股骨头坏死通常产生患侧盆下型骨盆倾斜，对全髋置换术中髋臼杯外展角有决定作用，随着倾斜程度加重，术中髋臼外展角应随之减小，骨盆倾斜越重，术后髋关节评分也越低。

목적：탐토고골두배사골분경사각도대전관관절치환술후관관절공능적영향。방법：50례환측분하형골분경사고골두배사환자균행전관관절치환술，수술전술후행X선검사，측정골분경사각、관구외전각화전경각，말차수방시평개관관절공능。결과：골분경사환자술후관구외전각평균위III형（37°）<II형（40°）<I형(44°，P＜0.05），말차수방관관절공능평분I형、II형명현고우III형（P＜0.05）。결론：고골두배사통상산생환측분하형골분경사，대전관치환술중관구배외전각유결정작용，수착경사정도가중，술중관구외전각응수지감소，골분경사월중，술후관관절평분야월저。
Objective To investigate the affecting avascular necrosis of femoral head after pelvic tilt angle on the hip joint function following total hip replacement. Methods A follow-up analysis on 50 cases of avascu?lar necrosis of the femoral head in total hip arthroplasty patients, both ipsilateral basin pelvic tilt, of which type 1 in 18 cases, type II in 16 cases, and type III in 6 cases was studied; with preoperative and postoperative X-ray examinations, determination of pelvic inclination angle, acetabular abduction and anteversion angle, hip joint function were assessed at the time of last follow-up. Results III pelvic tilt postoperative X-ray measure?ment of acetabular abduction (EVE joy, 37) <II (40 <I Eve joy,) (EVE joy, 44) showed there were significant dif?ferences between the groups, but the difference between the anteversion groups was not statistically significant, the last follow-up hip function score of I type、II type >III type, the difference was statistically significant. Conclusion Following avascular necrosis of the femoral head ipsilateral basin pelvic tilt is usually produced, showing a decisive role on the total hip replacement acetabular cup abduction angle of tilt with aggravation of in?traoperative acetabular abduction the abduction angle should decrease, III type I type 7 less than Eve joy, never?theless, more heavy the pelvic tilt, a lower postoperative hip score.
