目的:探讨幽门螺杆菌( HP)感染和慢性阻塞性肺疾病( COPD)的关系。方法通过174例COPD患者和185例健康人群的14 C尿素呼气试验,比较两组HP感染的阳性率。比较174例COPD患者中HP感染阳性和阴性的两组患者的肺功能和血气分析情况。结果 COPD组的HP阳性率为69/174例(39.66%),健康人群为56/185例(30.27%),两组比较差异有统计学意义(P<0.05)。肺功能检测结果HP感染阳性的COPD患者第1秒最大呼气量(FEV1)、用力肺活量( FVC)及FEV 1/FVC值明显低于HP阴性的COPD患者;血气分析结果为HP阳性COPD组的血氧分压( PaO 2)和血氧饱和度( SaO 2)水平低于阴性的COPD患者,而PaCO 2水平高于阴性的COPD患者。结论 COPD组的HP感染率高于正常人群组,HP感染与COPD的病情严重程度密切相关。

Objective To discuss the relationship between helicobacter pylori( HP) infection and chronic obstructive pul-monary disease(COPD). Methods To compare the rate of HP infection in 174 patients with COPD and 185 healthy people through the 14C-urea breath testl;To compare the results of lung function and blood gas analysis in the 174 COPD patients with HP ( +)and HP(-). Results The rate of HP infection was 69/174(39. 66)in COPD group vs. 56/185(30. 27%)in healthy peo-ple, there was significant difference between the two groups. Pulmonary function tests showed the FEV 1 , FVC, FEV 1/ FVC in COPD patients with HP( +)was significantly lower than in the COPD patients with HP(-). Blood gas analysis showed that the level of PaO 2 and SaO 2 was lower in the COPD patients with HP( +) than in the COPD patients with HP(-) ,and the COPD pa-tients with HP( +) had a higher PaCO 2 level. Conclusion HP infection rate is higher in the COPD group than in the general pop-ulation, HP infection is closely related to disease severity of COPD.
