目的:了解新生儿缺氧缺血性脑病( HIE)患儿血清神经元特异性烯醇化酶( NSE)的浓度变化及临床意义。方法选取41例足月HIE患儿及同期出生14例无窒息史、无神经系统,疾病史的足月新生儿作为研究对象,HIE组分为轻、中、重三组。使用电化学发光法测定血清NSE浓度。结果同一日龄HIE中重度组血清NSE含量均高于对照组,HIE中度组血清NSE含量均高于轻度组,重度组血清NSE含量高于中度组,差异均有统计学意义(P<0.05),HIE轻度组血清NSE含量高于对照组,但差异无统计学意义(P>0.05)。动态检测结果显示,血清NSE含量在伤后第1d达到峰值,然后缓慢下降。结论 NSE是判断HIE及损伤程度比较客观的指标,其含量越高,表示脑损伤程度越重,可作为HIE诊断及其分度的补充依据。

목적:료해신생인결양결혈성뇌병( HIE)환인혈청신경원특이성희순화매( NSE)적농도변화급림상의의。방법선취41례족월HIE환인급동기출생14례무질식사、무신경계통,질병사적족월신생인작위연구대상,HIE조분위경、중、중삼조。사용전화학발광법측정혈청NSE농도。결과동일일령HIE중중도조혈청NSE함량균고우대조조,HIE중도조혈청NSE함량균고우경도조,중도조혈청NSE함량고우중도조,차이균유통계학의의(P<0.05),HIE경도조혈청NSE함량고우대조조,단차이무통계학의의(P>0.05)。동태검측결과현시,혈청NSE함량재상후제1d체도봉치,연후완만하강。결론 NSE시판단HIE급손상정도비교객관적지표,기함량월고,표시뇌손상정도월중,가작위HIE진단급기분도적보충의거。
Objective To determine the concentration of serum neuron pecific enolase( NSE) as a specific marker of nerve injury in neonatol with hypoxic ischemic encephalopathy( HIE) and study clinical significance of NSE. Methods 41 patients with HIE were included and 14 healthy volunteers were recruited as controls in the study. According to clilic symptom, HIE patients were divided into three groups. Serum NSE level was determined by using ECISA. Results At the same day of age,the serum lev-els of NSE in patients with moderate and severe HIE were raised significantly as compared to control group(P<0. 05). Similarly,the levels of NSE in severe HIE group were obviously higher than those in moderate HIE group(P<0. 05). The levels of NSE in mod-erate HIE group were obviously higher than those in mild HIE group(P<0. 05). The serum levels of NSE reached the peak at the day 1 and then decreased slowly. Conclusion The levels of NSE are closely correlated with severity of HIE. Serum NSE may be used as a promising marker for diagnosis and evaluating severity of severe HIE.
