目的:探讨一步法HBV-DNA定量与乙肝血清标志物定量及肝功能相关生化指标的相关性,为临床乙肝诊断治疗提供依据。方法利用一步法荧光定量PCR检测乙肝病毒核糖核酸,时间分辨免疫荧光分析测定乙肝血清标志物,采用速率法检测肝功能相关生物指标。结果不同乙肝阳性模式,HBV-DNA的含量和阳性率有显著性差异。HBV-DNA与ALT水平无明显相关性,但与HBeAg含量成显著相关。同时,ALT与HBcAb成正相关,HBsAg含量与 A/G比值成负相关,HBeAg含量与TBil和DBil成正相关。结论一步法HBV-DNA检测,提高了乙肝病毒复制量检测的特异性和灵敏度,反映了病毒复制活跃情况和传染性强弱。乙肝表面抗原以及E抗原血清水平的强弱,与肝脏部分代谢存在一定相关性。因此,要分析判断乙肝病情、用药情况以及治疗效果,临床上应综合分析包括HBV-DNA,乙肝血清标志物以及肝功能各生化指标等在内的诸多因素。

ABSTRACT:Objective To intestate the associations among HBV-DNA quantitative and HBV serum markers and biochemi-cal ifgures of liver function, and provide evidence for hepatitis B clinic diagnosis and therapy. Methods One-step real-time PCR was used for HBV-DNA tests, TRFIA was used for analysis the serum markers of HBV, and continuous monitoring assay was used for analysis the biochemical ifgures of liver function. Results The serum levels and the positive rates of HBV-DNA were signiifcantly different among positive HBV makers groups. HBV-DNA levels showed no any linkage to ALT, however strong associated with HBeAg concentration. Correlation analyses showed that ALT positively associated with HBcAb, serum HBsAg concentration negatively associated with A/G ratio, and HBeAg concentration positively associated with TBil and DBil. Conclu-sion One-step HBV-DNA analyses showed higher speciifcity and sensitivity, its levels implied the activity degree of HBV copies and infection abilities. Serum HBsAg and HBeAg concentrations were correlated to partial biochemical ifgures of liver function. Therefore, clinic should take considered multiple factors, such as HBV-DNA, serum markers quantitative of HBV and biochemi-cal ifgures of liver function, for hepatitis B clinic diagnosis and drug therapy judgment.
