目的:探讨采用自制体位架拍摄腰椎功能位平片在掌控腰椎伸屈角度中的应用价值。方法应用自制的腰椎功能位摄片体位架,设置腰椎伸屈角度的一种测试方法,以受检者胸骨体与骶骨下部的连线与垂直面的夹角为伸位伸展角度α1,以耻骨联合处与肩胛骨的连线与垂直面的夹角为屈位屈曲角度α2,通过调节横杆装置X1、X2及垂直装置Y1、Y2的不同数值,用函数arctg[(X1+X2-50)/(Y1-Y2)]×360/2π分别计算出α1和α2。随机选择178例腰腿痛和坐骨神经痛的患者采用最大限度的过伸过屈位,根据X1、X2及Y1、Y2数值得出相应α1和α2。结果①设置横杆装置X1、X2及垂直装置Y1、Y2的不同数值,得出对应的α1和α2,编制腰椎功能位伸屈角度的查对表。②178例患者采用自制体位架拍摄腰椎功能位平片,测试腰椎最大限度的伸屈角度,用均值±标准差的形式对总体进行点估计,α1为33.98±13.97度;α2为34.78±14.99度,两者均符合正态分布。结论自制体位架通过水平和垂直测量装置能够掌控腰椎功能位摄片的伸屈角度,避免了盲目伸屈和主观随意性,方便准确,实用性较强。

Objective To explore the application value of self-made posture frame in controlling flexion angle in shooting lumbar function radiography.Methods Homemade position frame was applied in lumbar function radiography,a test was set for lumbar flexion an-gle;at an angle of subject body of sternum and sacrum bottom line and a vertical surface was extension angleα1,at an angle of pubic symphy-sis and scapular line and a vertical surface was flexion angleα2 ,and flexion angleα1 andα2 was calculated through the function arctg[ ( X1 +X2 -50)/( Y1 -Y2 ) ] ×360/2 π.A total of 178 cases of lumbocrural pain and sciatica patients were randomly selected using maximum flex-ion,and the correspondingα1 andα2 were obtained according to the X1 ,X2 and Y1 ,Y2 .Results ①By setting different numerical bar de-vice X1 ,X2 and vertical deviceY1 ,Y2 ,the corresponding α1 and α2 were obtained,and a check table of lumbar function extension and flexion angle was compiled.② By using self-made posture frame in shooting lumbar function radiography among 178 patients,the maxi-mum flexion angle was tested,point estimates of total x ±s,α1 was 33.98 ±13.97 degrees,and α2 was 34.78 ±14.99 degrees,both were in line with the normal distribution.Conclusion Position frame made by horizontal and vertical measuring device can control the ex-tension and flexion angle of the function of lumbar vertebrae radiographs,avoid blind flexion extension and subjectivity,and thus is conven-ient,accurate and practical.
