目的探讨糖尿病肾病(DN)患者同型半胱氨酸水平(Hcy)及其与炎症因子的关系。方法在本院就诊的2型糖尿病(T2DM )的患者120例,按DN不同进展期分为:DM正常白蛋白尿组(DM A组);早期糖尿病肾病组(DMB组);临床糖尿病肾病组(DMC组)各40例。另以同期健康体检正常者40例为对照组。检测四组研究对象血浆 Hcy、高敏 C 反应蛋白(hs‐CRP)、肿瘤坏死因子‐α(TNF‐α)、白介素‐6(IL‐6)水平。结果与健康对照组比较,DN 患者的血浆 Hcy水平、炎症因子指标明显升高,差异有统计学意义( P <0.01);随着DN尿蛋白的升高,血浆 Hcy及hs‐CRP、TNF‐α、IL‐6水平显著递增,差异有统计学意义( P <0.01);DN患者血浆Hcy升高与hs‐CRP、TNF‐α、IL‐6的水平呈正相关( r分别为0.420、0.461、0.429,P <0.01)。结论高Hcy血症及炎症因子参与了DN的发生和发展过程,Hcy可能通过促进机体炎症状态的产生促进DN的发生和发展。

목적탐토당뇨병신병(DN)환자동형반광안산수평(Hcy)급기여염증인자적관계。방법재본원취진적2형당뇨병(T2DM )적환자120례,안DN불동진전기분위:DM정상백단백뇨조(DM A조);조기당뇨병신병조(DMB조);림상당뇨병신병조(DMC조)각40례。령이동기건강체검정상자40례위대조조。검측사조연구대상혈장 Hcy、고민 C 반응단백(hs‐CRP)、종류배사인자‐α(TNF‐α)、백개소‐6(IL‐6)수평。결과여건강대조조비교,DN 환자적혈장 Hcy수평、염증인자지표명현승고,차이유통계학의의( P <0.01);수착DN뇨단백적승고,혈장 Hcy급hs‐CRP、TNF‐α、IL‐6수평현저체증,차이유통계학의의( P <0.01);DN환자혈장Hcy승고여hs‐CRP、TNF‐α、IL‐6적수평정정상관( r분별위0.420、0.461、0.429,P <0.01)。결론고Hcy혈증급염증인자삼여료DN적발생화발전과정,Hcy가능통과촉진궤체염증상태적산생촉진DN적발생화발전。
[Objective] To explore the correlation between homocysteine(Hcy) and inflammatory factors in patients with diabetic nephropathy(DN) .[Methods]According to DN progression ,120 patients diagnosed as type 2 diabetes mellitus(T2DM) in our hospital were divided into DM with normal proteinuria group(DMA group) ,early DN group(DMB proup) and clinical DN group(DMC group) with 40 patients in each group .In addition ,40 healthy people were chosen as the control group .The levels of Hcy and inflammatory factors(hs‐CRP ,TNF‐α,IL‐6) in 4 groups were detected .[Results]Compared with the control group ,the levels of Hcy and inflammatory factors in DN group were obviously increased ,and there was significant difference( P <0 .01) .With the increasing of urine proteins in DN ,the levels of Hcy ,hs‐CRP ,TNF‐αand IL‐6 were signifi‐cantly increased by degrees ,and there was significant difference( P<0 .01) .The increasing of Hcy in DN pa‐tients was positively correlated with hs‐CRP ,TNF‐αand IL‐6( r =0 .420 ,0 .461 ,0 .429 ,P <0 .01) .[Con‐clusion]Hyperhomocysteinaemia and inflammatory factors may involve in the occurrence and development of DN .Hcy may promote the occurrence and development of DN through promoting body inflammatory status .
