目的:探讨高频超声检查在肘管综合征(CTS)诊断中的价值。方法经临床拟诊并手术证实的20例(共20个肘管)CTS 患者,术前采用高频超声检查尺神经走行、形态、回声、肿胀程度、卡压原因及部位,将手术与超声检查结果进行对比。结果超声声像:20例中60%(12/20)可见明显卡压;卡压原因为骨质增生(骨赘)、骨化性肌炎、囊肿、瘢痕、腱性增生,卡压处尺神经局部变细,卡压近端远段肿胀明显,回声减低;40%(8/20)未见明显卡压,仅表现为肘管尺神经肿胀。与手术结果对比,超声检出神经肿胀病变符合率为100%,卡压检出率为60%。结论高频超声能够显示肘管结构、尺神经卡压性病变及部分卡压原因和部位,可为临床 CTS 的诊治提供参考。
目的:探討高頻超聲檢查在肘管綜閤徵(CTS)診斷中的價值。方法經臨床擬診併手術證實的20例(共20箇肘管)CTS 患者,術前採用高頻超聲檢查呎神經走行、形態、迴聲、腫脹程度、卡壓原因及部位,將手術與超聲檢查結果進行對比。結果超聲聲像:20例中60%(12/20)可見明顯卡壓;卡壓原因為骨質增生(骨贅)、骨化性肌炎、囊腫、瘢痕、腱性增生,卡壓處呎神經跼部變細,卡壓近耑遠段腫脹明顯,迴聲減低;40%(8/20)未見明顯卡壓,僅錶現為肘管呎神經腫脹。與手術結果對比,超聲檢齣神經腫脹病變符閤率為100%,卡壓檢齣率為60%。結論高頻超聲能夠顯示肘管結構、呎神經卡壓性病變及部分卡壓原因和部位,可為臨床 CTS 的診治提供參攷。
목적:탐토고빈초성검사재주관종합정(CTS)진단중적개치。방법경림상의진병수술증실적20례(공20개주관)CTS 환자,술전채용고빈초성검사척신경주행、형태、회성、종창정도、잡압원인급부위,장수술여초성검사결과진행대비。결과초성성상:20례중60%(12/20)가견명현잡압;잡압원인위골질증생(골췌)、골화성기염、낭종、반흔、건성증생,잡압처척신경국부변세,잡압근단원단종창명현,회성감저;40%(8/20)미견명현잡압,부표현위주관척신경종창。여수술결과대비,초성검출신경종창병변부합솔위100%,잡압검출솔위60%。결론고빈초성능구현시주관결구、척신경잡압성병변급부분잡압원인화부위,가위림상 CTS 적진치제공삼고。
Objective To investigate the value of high frequency ultrasound in the diagnosis of cubital tunnel syndrome (CTS).Methods High frequency ultrasound was used preoperatively to measure the course,shape,echo,swelling degree and compression cause and location of ulnar nerve in 20 patients with CTS confirmed by surgery (20 elbows).Ultrasonic findings were com-pared with surgical results.Results Among the 20 patients,obvious ulnar nerve compression was observed in 12 (60%).The causes of compression included bone hyperplasia (osteophyte),myosi-tis ossificans,cyst,scar and tendinous hyperplasia.Local thinning was found in ulnar nerve at the place of compression with proximal swelling and echo reduction.Eight (40%)of the 20 patients had no obvious compression and only showed ulnar nerve swelling at the cubital tunnel.Compared with surgical results,high frequency ultrasound provided a coincidence rate of 100% for nerve swelling lesions and a detection rate of 60% for compression.Conclusion High frequency ultra-sound can display the structure of cubital tunnel,compression lesions of ulnar nerve,and cause and location of compression.Therefore,high frequency ultrasound may provide the basis for clini-cal diagnosis and treatment of CTS.
