目的:探讨反复呼吸道感染哮喘患儿血清维生素 A水平,了解其与免疫功能的相关性。方法选取2012年7月至2013年2月间于郴州市第一人民医院儿童医院呼吸内科就诊并治疗的反复呼吸道感染患儿72例(呼吸道感染组)及哮喘患儿25例(哮喘组),同时选取40例健康小儿为对照组,对比分析3组间 VitA 水平及免疫指标间的差异,同时进行相关性分析。结果与对照组相比,反复呼吸道感染患儿及哮喘患儿的 VitA水平均显著下降(P<0.05);对于免疫指标的检查,与对照组相比,反复呼吸道感染患儿除 IgM水平外,其余各项指标间差异均具有统计学意义(P<0.05),而对于哮喘患儿而言,仅补体 C3水平间的差异具有统计学意义(P<0.05);经 Pearson相关性分析发现,反复呼吸道感染患儿血清 VitA 与 IgA、CD3+细胞数、CD3+CD4+细胞数、CD4+/CD8+比例间存在正相关性,与 CD3+CD8+细胞数目间呈现负相关性;而对于哮喘患儿而言,血清 VitA与各项免疫指标间均未发现明显的相关性。结论 VitA的缺乏可能导致机体免疫功能的减弱,降低儿童的免疫力,其可能是造成反复呼吸道感染及哮喘发病的重要原因,临床中应加强对于 VitA的检查。

목적:탐토반복호흡도감염효천환인혈청유생소 A수평,료해기여면역공능적상관성。방법선취2012년7월지2013년2월간우침주시제일인민의원인동의원호흡내과취진병치료적반복호흡도감염환인72례(호흡도감염조)급효천환인25례(효천조),동시선취40례건강소인위대조조,대비분석3조간 VitA 수평급면역지표간적차이,동시진행상관성분석。결과여대조조상비,반복호흡도감염환인급효천환인적 VitA수평균현저하강(P<0.05);대우면역지표적검사,여대조조상비,반복호흡도감염환인제 IgM수평외,기여각항지표간차이균구유통계학의의(P<0.05),이대우효천환인이언,부보체 C3수평간적차이구유통계학의의(P<0.05);경 Pearson상관성분석발현,반복호흡도감염환인혈청 VitA 여 IgA、CD3+세포수、CD3+CD4+세포수、CD4+/CD8+비례간존재정상관성,여 CD3+CD8+세포수목간정현부상관성;이대우효천환인이언,혈청 VitA여각항면역지표간균미발현명현적상관성。결론 VitA적결핍가능도치궤체면역공능적감약,강저인동적면역력,기가능시조성반복호흡도감염급효천발병적중요원인,림상중응가강대우 VitA적검사。
Objective To investigate the serum level of vitamin A in children with recurrent re-spiratory tract infections or intrinsic asthma,and to understand its correlation with immune func-tion.Methods Seventy-two children with recurrent respiratory tract infections and 2 5 children with intrinsic asthma who were treated in the Department of Respiratory Medicine of Chenzhou First People’s Hospital between July 2012 and February 2013 were selected in this study.In addi-tion,40 healthy children were selected as controls.Differences in serum vitamin A level and im-mune indices were compared among the three groups.The correlation between vitamin A level and immune function was analyzed.Results Compared with controls,serum vitamin A level signifi-cantly decreased in children with recurrent respiratory tract infections or intrinsic asthma (P<0.05).Furthermore,there were significant differences in immune indices,except IgM,between controls and children with recurrent respiratory tract infections (P<0.05 ).However,only C3 complement was obviously different between controls and children with intrinsic asthma (P<0.05).Pearson correlation analysis showed that serum vitamin A level was positively correlated with IgA,number of CD3+ cells,number of CD3+ CD4+ cells and ratio of CD4+/CD8+,but nega-tively correlated with the number of CD3+CD8+ cells in children with recurrent respiratory tract infections.No obvious correlation was found between vitamin A level and immune indices in chil-dren with intrinsic asthma.Conclusion The deficiency of vitamin A may lead to an impairment of immune function in children,which may be an important cause of recurrent respiratory tract infec-tions and intrinsic asthma.Therefore,clinical examination of serum vitamin A level should be strengthened.
