目的：探讨偏远地区对胎盘早剥的高危因素及对母婴的影响和诊治的体会。方法分析我院近1年胎盘早剥的发病情况、产前检查情况和母婴结局。结果在851例住院分娩产妇中，胎盘早剥12例，发生率为1.4%。12例病例中8例孕28周后发病患者5例合并妊娠期高血压；2例合并慢性高血压；另4例孕28周前发病患者中3例有同房史，1例有外伤史。12例均未规范产检，均为柯族人，2例为初产妇，10例为经产妇。结论预防胎盘早剥发病的关键是加强产前检查，及早治疗妊娠期高血压等疾病。对阿合奇这样偏远地区的牧民加强孕期保健及宣教，对降低孕产妇和围产儿病死率尤为重要。

Objective To explore highly pathogenetic factors of placental abruption, its inlfuences on mother and baby and its treatments in remote districts. Methods Do analysis on placental abruption morbidity, diagnosis before birth and mother&baby results in our hospital within one year. Results In 851 cases of delivery women, there were 12 cases of placental abruption, accounted for 1.4%. Among these 12 cases, 8 cases happened after 28 weeks of pregnancy while 5 cases had hypertension during pregnancy;2 cases had chronic hypertension;besides, 3 of the 4 cases of morbidity 28 weeks of pregnancy ago had sex while the other one had the history of injury. All of 12 patients of Ke nation in the placental abruption cases didn’t do standard birth check. Among them, 2 cases were primiparas while the other 10 cases weren’t. Conclusions The key to prevent placental abruption is to do before-birth checks strictly and to treat the diseases such as hypertension during pregnancy in time. In order to decrease the death rate of mother and baby caused by placental abruption, it is critical to do pregnancy propaganda and education for the herdsmen in the remote district like Akeqi.
