目的:比较上肢不同注射部位对胸部CT增强扫描中对比剂利用效果的影响。方法选择76例接受CT胸部增强扫描患者,均采用屈肘位(仰卧位,双上臂上举,双前臂交叉置于额头上,留置静脉通道的手臂在上),双筒高压注射对比剂。根据注射部位的不同分为右侧屈肘组(仰卧位,双上臂上举,双前臂交叉置于额头上,留置静脉通道的右手臂在上)和左侧屈肘组(仰卧位,双上臂上举,双前臂交叉置于额头上,留置静脉通道的左手臂在上)。测量双锁骨下静脉胸廓入口内外各2 cm~3 cm处CT值,取其平均值的差值作为评价对比剂利用效果的指标。结果两种不同注射部位之间比较,对比剂的利用效果有一定差异。结论胸部CT增强扫描时,采用右侧屈肘位注射有助于降低腋静脉及锁骨下静脉内对比剂在胸廓外的留滞,提高对比剂的利用率,减少对比剂局部滞留引起的伪影。

목적:비교상지불동주사부위대흉부CT증강소묘중대비제이용효과적영향。방법선택76례접수CT흉부증강소묘환자,균채용굴주위(앙와위,쌍상비상거,쌍전비교차치우액두상,류치정맥통도적수비재상),쌍통고압주사대비제。근거주사부위적불동분위우측굴주조(앙와위,쌍상비상거,쌍전비교차치우액두상,류치정맥통도적우수비재상)화좌측굴주조(앙와위,쌍상비상거,쌍전비교차치우액두상,류치정맥통도적좌수비재상)。측량쌍쇄골하정맥흉곽입구내외각2 cm~3 cm처CT치,취기평균치적차치작위평개대비제이용효과적지표。결과량충불동주사부위지간비교,대비제적이용효과유일정차이。결론흉부CT증강소묘시,채용우측굴주위주사유조우강저액정맥급쇄골하정맥내대비제재흉곽외적류체,제고대비제적이용솔,감소대비제국부체류인기적위영。
Objective To compare the impact of upper limbs injection positions on utilization of contrast agent in contrast-enhanced thoracic CT scanning. Methods Totally 76 patients underwent contrast-enhanced thoracic CT examination. All the patients were elbow flexion position (supine position, the patient's double arm lifted, forearms crossing on forehead , with the intravenous route side of arm above the other side) and double tube high pressure injection of contrast agent. Then the patients were divided into 2 groups according to upper limbs injection positions. The first position group was right elbow flexion position, while the second position group was left elbow flexion position. The difference between mean CT values at 2-3 cm of right subclavian vein inside and outside of the entrance of thoracic cage was measured and regarded as the evaluation index of utilization of contrast agent. Result There was significant difference in utilization of contrast agent between the two injection positions. Conclusion During contrast-enhanced thoracic CT scanning,taking the right elbow flexion position can decrease the accumulation of contrast agents within the axillary vein and subclavian vein outside the chest,which helps to improve the utilization of contrast agent and reduce local artifacts caused by accumulation of contrast agent.
