目的：探讨血清脂联素（ APN）及C反应蛋白（ CRP）与APN比值（ CRP／APN比值）的变化在腹部创伤患者病情监测中的意义。方法将本科室收治的80例腹部外伤患者依据AIS－ISS评分分为轻伤组及重伤组，将来本院体检的正常成年人30例作为对照组。分别于入院后即刻及第1 d、2 d、4 d、7 d共5个时间点，采用放射免疫法检测患者APN、CRP水平及CRP／APN比值，并同步进行全身炎症反应综合征（ SIRS）评分，对所得结果进行统计学分析及相关性分析。结果与对照组相比，轻伤组和重伤组患者的APN水平均明显降低（ P＜0.05），重伤组APN水平明显低于轻伤组（ P＜0.05）；血清CRP与CRP／APN比值与SIRS评分呈正相关，相关系数分别为0.81（P＜0.05）和0.91（P＜0.05），而APN水平与SIRS评分呈负相关（r＝－0.88，P＜0.05）。结论APN与CRP／APN比值可以作为评估腹部伤情严重程度的客观指标，有助于准确及时地评估病情。
목적：탐토혈청지련소（ APN）급C반응단백（ CRP）여APN비치（ CRP／APN비치）적변화재복부창상환자병정감측중적의의。방법장본과실수치적80례복부외상환자의거AIS－ISS평분분위경상조급중상조，장래본원체검적정상성년인30례작위대조조。분별우입원후즉각급제1 d、2 d、4 d、7 d공5개시간점，채용방사면역법검측환자APN、CRP수평급CRP／APN비치，병동보진행전신염증반응종합정（ SIRS）평분，대소득결과진행통계학분석급상관성분석。결과여대조조상비，경상조화중상조환자적APN수평균명현강저（ P＜0.05），중상조APN수평명현저우경상조（ P＜0.05）；혈청CRP여CRP／APN비치여SIRS평분정정상관，상관계수분별위0.81（P＜0.05）화0.91（P＜0.05），이APN수평여SIRS평분정부상관（r＝－0.88，P＜0.05）。결론APN여CRP／APN비치가이작위평고복부상정엄중정도적객관지표，유조우준학급시지평고병정。
Objective To investigate the clinical significance of serum adiponectin and ratio of CRP /APN in evaluating the illness state for abdominal trauma patients.Methods 80 patients were divided into mild trauma group and severe trauma group according to AIS -ISS score.And healthy adults who had physical examination in the hospital were regarded as the observation group .Concentration of adiponectin and CRP were detected when they were admitted to emergency room as well as 1nd,2th,4th and 7th day in the morning.Meanwhile, ration of CRP/APN and SIRS scoring were assessed .Statistic analysis and correlation analysis were performed .Rseults Compared with normal group ,the APN levels of mild trauma group and severe trauma group were lower ( P<0.05) ,while APN level of severe trauma group was significantly lower (P<0.05).Serum levels of CRP and CRP/APN ratio were positive correlated with SIRS score system ,and correlation coefficiency was0.81 (P<0.05) and 0.91 (P<0.05) respectively,while APN was negative correlated with SIRS score system (r=-0.88 ,P<0.05 ).Con clusion Concentration of APN and CRP/APN ratio could be objective index for evaluation of severity and prognosis in abdominal traumatic patients , and have a good predictive value on the severity of abdominal trauma.
