目的:研究血清多种肿瘤标志物联合检测在肺癌诊断中的价值,寻找最为有效的肺癌诊断方式。方法:选取自2012年5月-2013年12月入住本院的200例肺癌患者进行癌胚抗原(CEA)、糖类抗癌原199(CA19-9)、糖类癌抗原125(CA125)、神经元特异性烯醇化酶(NSE)和细胞角蛋白片段19(CYFRA21-1)等5个项目的敏感度、准确性和特异度检测。血清癌胚抗原(CEA)、细胞角蛋白片段19(CYFRA21-1)、神经元特异性烯醇化酶( NSE)、糖类抗癌原199(CA19-9)和糖类抗癌抗原125(CA125)均采用化学发光法进行检测,将检测的结果进行统计学数据处理。结果:细胞角蛋白片段19(CYFRA21-1)在单个项目的检测中肺癌诊断的敏感度和准确性最高为(64%和75.6%),然而特异度却是糖类抗癌原199(CA19-9)以(98.8%)排在首位。与单个项目的检测数据相对比联合检测的敏感度和准确性都有着明显的提高,以神经元特异性烯醇化酶(NSE)和细胞角蛋白片段19(CYFRA21-1)(83.6%和82.9%)排在第一,特异度则以糖类癌抗原125(CA125)和糖类抗癌原199(CA19-9)(97.6%)最高。结论:通过结果显示联合检测在肺癌诊断中的敏感度、准确性都有着明显的提高,极大的提高的肺癌诊断的准确率,可以广泛的应用于临床中。

목적:연구혈청다충종류표지물연합검측재폐암진단중적개치,심조최위유효적폐암진단방식。방법:선취자2012년5월-2013년12월입주본원적200례폐암환자진행암배항원(CEA)、당류항암원199(CA19-9)、당유암항원125(CA125)、신경원특이성희순화매(NSE)화세포각단백편단19(CYFRA21-1)등5개항목적민감도、준학성화특이도검측。혈청암배항원(CEA)、세포각단백편단19(CYFRA21-1)、신경원특이성희순화매( NSE)、당류항암원199(CA19-9)화당류항암항원125(CA125)균채용화학발광법진행검측,장검측적결과진행통계학수거처리。결과:세포각단백편단19(CYFRA21-1)재단개항목적검측중폐암진단적민감도화준학성최고위(64%화75.6%),연이특이도각시당류항암원199(CA19-9)이(98.8%)배재수위。여단개항목적검측수거상대비연합검측적민감도화준학성도유착명현적제고,이신경원특이성희순화매(NSE)화세포각단백편단19(CYFRA21-1)(83.6%화82.9%)배재제일,특이도칙이당유암항원125(CA125)화당류항암원199(CA19-9)(97.6%)최고。결론:통과결과현시연합검측재폐암진단중적민감도、준학성도유착명현적제고,겁대적제고적폐암진단적준학솔,가이엄범적응용우림상중。
Objective: To investigate the serum tumor markers in diagnosis of lung cancer, find the most effective way for the diagnosis of lung cancer.Method: 200 patients with lung cancer from 2012 May to 2013 December were selected in our hospital for carcinoembryonic antigen (CEA), carbohydrate anticarcinogens 199 (CA19-9), carbohydrate antigen 125 (CA125 cancer), neuron specific enolase (NSE) and cytokeratin 19 (CYFRA21-1) sensitivity, accuracy and specificity of detection of 5 items. Serum carcinoembryonic antigen (CEA), cytokeratin 19 (CYFRA21-1), neuron specific enolase (NSE), carbohydrate anticarcinogens 199 (CA19-9) and carbohydrate antigen 125 (CA125) cancer were detected by chemiluminescence method, the result would be detected for statistical data processing.Result: The expression of cytokeratin 19 (CYFRA21-1) sensitivity and accuracy in the diagnosis of lung cancer in the detection of single item in the highest (64% and 75.6%), but the specificity was carbohydrate anticarcinogens 199 (CA19-9) to (98.8%) in the first row. The testing data and the single project relative sensitivity and accuracy of combined detection had significantly increased, the neuron specific enolase (NSE) and cytokeratin 19 (CYFRA21-1) of (83.6% and 82.9%) in the first row, while the carbohydrate specificity of cancer antigen 125 (CA125) and carbohydrate anticarcinogens 199 (CA19-9) (97.6%) the highest.Conclusion: The results show the sensitivity, accuracy of joint detection in the diagnosis of lung cancer have significantly improved the accuracy of diagnosis of lung cancer, the great, can be widely applied to clinical.
