目的:探讨血、尿胱抑素C(CYS)在不同时期肾综合征出血热(HERS)急性肾损伤(AKI)检测的价值。方法:选取2010年3月-2012年10月期间本院确诊治疗的HERS患者36例作为HERS组,依据临床分期分为发热期/低血压休克期、少尿期、多尿期/恢复期,依据病情严重程度分为轻型组、中型组、重型组,选取同期体检中心健康人35名作为正常组。对所有患者通过酶法测定血清肌酐(CREA)和酶联免疫吸附法测CYS,采用Pearson相关性分析评价两者相关性,统计分析所有受试者患者血、尿CYS水平对AKI检测的价值。结果:在发热/低血压休克期、少尿期,HERS组患者中血、尿CYS、CREA水平明显高于正常组,差异有统计学意义(P<0.05);在发热/低血压休克期、少尿期血、尿CYS水平方面:轻型组>中型组>重型组,差异有统计学意义(F1=4.80,F2=5.92,P<0.05);Pearson相关性分析结果显示,血、尿CYS水平与CREA水平呈现正相关(r1=0.813,P<0.001;r2=0.743,P<0.001)。结论:血、尿CYS水平与不同时期HERS患者肾功能存在密切联系,可更为有效反映患者AKI病情,可作为患者AKI检测的重要指标,值得临床作进一步推广。

목적:탐토혈、뇨광억소C(CYS)재불동시기신종합정출혈열(HERS)급성신손상(AKI)검측적개치。방법:선취2010년3월-2012년10월기간본원학진치료적HERS환자36례작위HERS조,의거림상분기분위발열기/저혈압휴극기、소뇨기、다뇨기/회복기,의거병정엄중정도분위경형조、중형조、중형조,선취동기체검중심건강인35명작위정상조。대소유환자통과매법측정혈청기항(CREA)화매련면역흡부법측CYS,채용Pearson상관성분석평개량자상관성,통계분석소유수시자환자혈、뇨CYS수평대AKI검측적개치。결과:재발열/저혈압휴극기、소뇨기,HERS조환자중혈、뇨CYS、CREA수평명현고우정상조,차이유통계학의의(P<0.05);재발열/저혈압휴극기、소뇨기혈、뇨CYS수평방면:경형조>중형조>중형조,차이유통계학의의(F1=4.80,F2=5.92,P<0.05);Pearson상관성분석결과현시,혈、뇨CYS수평여CREA수평정현정상관(r1=0.813,P<0.001;r2=0.743,P<0.001)。결론:혈、뇨CYS수평여불동시기HERS환자신공능존재밀절련계,가경위유효반영환자AKI병정,가작위환자AKI검측적중요지표,치득림상작진일보추엄。
Objective:To discuss the value of blood and urine bladder inhibition C(CYS)at different times by hemorrhagic fever of renal syndrome(HERS)of acute kidney injury(AKI)detection.Method:36 patients with HERS were selected in our Hospitals as HERS group,accorded to the clinical stage can be divided into hot/hypotension shock stage,oliguria stage,urine/recovery stage;accorded to the illness severity were divided into light group,medium group and heavy group.35 cases from health physical examination center personnel were selected for the same period as the normal group.All cases were used enzymatic determination of serum creatinine(CREA)and enzyme linked immunosorbent CYS,by Pearson correlation analysis and evaluation both correlation,analysised of CYS blood and urine levels of all the subjects with the value of the detection of AKI.Result:HERS group patients in fever/hypotension shock stage,oliguria stage,blood and urine of CYS and CREA level were obviously higher than that of normal group , the differences were statistically significant(P<0.05);fever/hypotension shock stage,oliguria stage CYS blood and urine levels:light group>medium group>heavy group,the differences were statistically significant(F1=4.80,F2=5.92, P<0.05);Pearson correlation analysis,accorded to the results of blood and urine levels of CYS positively related with CREA level(r1=0.813,P<0.001;r2=0.001,P <0.001).Conclusion:Blood and urine levels of CYS patients with HERS at different periods of renal function is closely related,can more effectively reflect AKI patient,can be used as an important index of testing in patients with acute kidney injury,it’s worth for further clinical promotion.
