目的：探讨吻合器痔上黏膜环切术（procedure for prolapse and hemorrhoids，PPH）联合外痔切除术治疗重度混合痔的临床疗效。方法：将2007年8月—2013年12月收治的84例重度混合痔患者随机分为研究组（PPH 联合外痔切除术）与对照组（传统外剥内扎术），每组各42例。对比分析2组患者的手术时间、住院时间、创面愈合时间，术后72 h 内出血、肛门疼痛、肛缘水肿、肛门溢液情况，以及术后6个月肛门狭窄的发生情况。结果：研究组的手术时间、住院时间、创面愈合时间明显短于对照组，研究组术后肛门疼痛、肛缘水肿、肛门溢液发生率低于对照组，差异均有统计学意义（P ＜0．05）。结论：PPH 联合外痔切除术是治疗重度混合痔的较佳术式。
목적：탐토문합기치상점막배절술（procedure for prolapse and hemorrhoids，PPH）연합외치절제술치료중도혼합치적림상료효。방법：장2007년8월—2013년12월수치적84례중도혼합치환자수궤분위연구조（PPH 연합외치절제술）여대조조（전통외박내찰술），매조각42례。대비분석2조환자적수술시간、주원시간、창면유합시간，술후72 h 내출혈、항문동통、항연수종、항문일액정황，이급술후6개월항문협착적발생정황。결과：연구조적수술시간、주원시간、창면유합시간명현단우대조조，연구조술후항문동통、항연수종、항문일액발생솔저우대조조，차이균유통계학의의（P ＜0．05）。결론：PPH 연합외치절제술시치료중도혼합치적교가술식。
Objective:To investigate the efficacy of the procedure for prolapse and hemorrhoids(PPH)combined with external hemorrhoidectomy in the treatment of severe mixed hemorrhoids.Methods:A total of 84 patients with severe mixed hemor-rhoids,who underwent surgical treatment from Aug 2007 to Dec 2013,were randomly divided into study group(received PPH combined with external hemorrhoidectomy)and control group(received Milligan-Morgan hemorrhoidectomy).The operation time,hospitalization time,wound healing time and bleeding,anal pain,edema of anal edge,anal leakage liquid within the 72 hours after surgery were compared between the two groups.The occurrence of anal stenosis within the 6 months after the operation were also compared.Results:The operation time,hospitalization time,wound healing time in the study group were significantly shorter than those in the control group.The Occurence rates of postoperative anal pain,edema of anal edge anal leakage liguid in the study group were lower than those in the control group and the differences were statistically significant(P <0.05).Con-clusions:PPH combined with external hemorrhoidectomy is the preferred treatment for severe mixed hemorrhoids.
