目的:研究胸部病灶18F-脱氧葡萄糖正电子发射断层扫描与CT断层扫描融合显像(18F-FDG?PET/CT)的标准摄取值(SUV)与磁共振单、多指数模型水分子扩散加权成像(DWI)水分子表观扩散系数(ADC)之间定量化关系,以揭示胸部病灶糖代谢与水分子扩散和转运关系。方法选择14例患者,临床病理诊断明确。计算并比较胸部病变SUV与单指数模型(b值0,?1000?s/mm2)、多指数模型(小b值:0?~?200?s/mm2、中b值:300?~?1500?s/mm2和高b值:1700?~?4500?s/mm2)的ADC值。结果良恶性病灶的SUV及单、多指数模型ADC值之间差异无统计学意义(P?>?0.05),提示仅根据SUV或ADC不能鉴别病灶的良恶性。14例患者胸部病灶SUV与单指数模型计算的ADC值之间无明显的相关性(r?=?-0.17),而与多指数模型高b值段计算的ADC值之间呈现明显的相关性(r?=?0.56)。结论 SUV与单指数模型计算的ADC值之间无相关性,而与多指数模型高b值段计算的ADC值之间呈现明显的相关性。这主要与病灶组织细胞在18F-FDG摄取增高的同时,病灶组织细胞水通道蛋白表达增高导致水分子在细胞膜的转运速度增高有关。

ObjectiveTo?investigate?the?quantitative?relationship?between?glucose?metabolism?and?water?molecules?diffusion?and?transport?by?studying?the?standardized?uptake?value(SUV)of?18F-FDG?PET/CT?and?apparent?diffusion?coefficient(ADC)of?diffusion-weighted?imaging(DWI).?Methods A?total?of?14?patients?with?pathological?diagnosis?were?recruited?for?the?study.?For?each?thoracic?lesion,?the?SUV,?ADC?calculated?with?mono-exponentia?model(b?=?0,?1000?s/mm2),?and?ADC?calculated?with?multiple-exponential?model(low?b,?0?~?200?s/mm2,?middle?b,?300?~?1500?s/mm2?and?high?b,?1700?~?4500?s/mm2)were?compared.?Results There?were?no?significant?differences?in?SUV?or?ADC?between?the?benign?and?malignant?lesions.These?results?indicated?that?SUV?or?ADC?alone?cannot?be?used?to?differentiate?lesions.?In?the?14?thoracic?lesions,?No?significant?correlation?was?found?between?SUV?and?the?ADC?calculated?with?mono-exponentia?mode(b?=?0,?1000?s/mm2)(r?=?-0.17).?However,?SUV?was?significantly?correlated?with?ADC?calculated?with?multiple-exponential?model(b,?1700?~?4500?s/mm2)(r?=?0.56).?Conclusion There?was?no?correlation?between?SUV?and?the?ADC?calculated?with?mono-exponentia?model(b?=?0,?1000?s/mm2).?However,?ADC?calculated?with?multiple-exponential?model(b,?1700?~?4500?s/mm2)was?correlated?with?SUV.?The?reason?may?be?that?when?the?uptake?of?18F-FDG?increased,?the?speed?of?water?molecules?transport?in?the?cell?membrane?was?also?increased?due?to?the?expression?of?water?channel?protein.
