目的:探讨256层CT扫描在门静脉成像的最佳时间点及分布区间,寻找门静脉成像的最佳方法。方法:对30例患者采用256层CT行上腹部灌注扫描,于注射造影剂5 s后行连续动态扫描,曝光时间间隔为3 s,观察门静脉成像全过程,选择最佳时相分布区间。以此为参考,在该区间设定多个定时曝光时间点,再对57例患者进行上腹部灌注扫描,于注射造影剂后延迟25 s间隔扫描,间隔时间为5 s,曝光4次,时间点为25、30、35、40 s。从四组门脉期图像中选取最佳时相进行门脉系统CTV图像重组。测量门静脉、肝脏CT值,并计算CT差值,评估门静脉成像质量。并分析门静脉最佳时间点的分布与受试者心率、体重及身高之间的相关性。结果:注射对比剂后25~40s 多时相点定时扫描,对包括95%的患者行门静脉最佳时相成像,门脉最佳时间点的分布与患者心率分布呈相关性(P<0.05),与患者身高、体重分布无相关性(P>0.05)。结论:256层螺旋CT多时相点定时扫描,是门静脉成像的可靠方案。

목적:탐토256층CT소묘재문정맥성상적최가시간점급분포구간,심조문정맥성상적최가방법。방법:대30례환자채용256층CT행상복부관주소묘,우주사조영제5 s후행련속동태소묘,폭광시간간격위3 s,관찰문정맥성상전과정,선택최가시상분포구간。이차위삼고,재해구간설정다개정시폭광시간점,재대57례환자진행상복부관주소묘,우주사조영제후연지25 s간격소묘,간격시간위5 s,폭광4차,시간점위25、30、35、40 s。종사조문맥기도상중선취최가시상진행문맥계통CTV도상중조。측량문정맥、간장CT치,병계산CT차치,평고문정맥성상질량。병분석문정맥최가시간점적분포여수시자심솔、체중급신고지간적상관성。결과:주사대비제후25~40s 다시상점정시소묘,대포괄95%적환자행문정맥최가시상성상,문맥최가시간점적분포여환자심솔분포정상관성(P<0.05),여환자신고、체중분포무상관성(P>0.05)。결론:256층라선CT다시상점정시소묘,시문정맥성상적가고방안。
Objective:To explore the best time point and the distribution range of the imaging of hepatic portal vein using 256-slice spiral CT perfusion technique,to find the best way in the hepatic portal vein scanning.Method:30 patients underwent upper abdominal scanning by 256-slice Spiral CT in the first. Done the continuous dynamic scanning delaying 5 s after injection of contrast agents,then observe the whole process of the hepatic portal imaging to get the best distribution range of exposure time. According to this distribution range to set exposure points, then 57 patients underwent upper abdominal scanning. Interval scanning were done delaying 25 s after injection of contrast agents, and the interval of exposure time was 5 s, then taken four times exposures, the time points were 25, 30, 35, 40 s. Elected the best phase from the four groups of hepatic portal vein imaging to reconstruct CTV. The CT value of hepatic, liver and calculated D-value to assess the quality of images were measured. the correlation between the best time point distribution of portal vein and heart rate, weight, height of subjects were analyzed.Result: Best images could be obtained in 95% patients using multitemporal timing scanning delaying 25-40 s after injecting of contrast agents. This result had correlation with patients’heart rate (P<0.05),but had no correlation with patients’ height and weight (P>0.05).Conclusion:Multitemporal timing scanning using 256-slice Spiral CT was another reliable method of hepatic portal vein imaging.
