目的：探讨膀胱癌患者尿液特异性核基质蛋白-4（ BLCA-4）的表达及临床意义。方法以80例初诊膀胱癌患者为研究对象，选择50例膀胱炎患者及50例健康体检者为对照。比较三组患者尿液中BLCA-4表达的差异及其与膀胱癌患者临床病理指标的关系。结果膀胱癌患者尿液中BLCA-4水平显著高于膀胱炎患者及健康体检者，差异具有统计学意义（P＜0．05），膀胱炎及健康体检者之间未见统计学差异（P＞0．05）；BLCA-4对膀胱癌的诊断灵敏度为93．8％，特异度为100．0％；BLCA-4与膀胱癌患者年龄、性别、病理分级、肿瘤大小及临床分期等因素均无相关性（ P＞0．05）；复发患者与未复发患者术前及术后6个月尿液中BLCA-4水平比较均无显著差别（P＞0．05），两组患者术后6个月BLCA-4水平均显著下降（P＜0．05），但术后1年6例复发患者尿液BLCA-4水平均显著高于术后6个月水平（P＜0．05），且与未复发患者相比差异具有统计学意义（P＜0．05）。结论 BLCA-4对膀胱癌诊断敏感度高，加强对膀胱癌患者术后BLCA-4水平的监测有助于预测复发情况。
목적：탐토방광암환자뇨액특이성핵기질단백-4（ BLCA-4）적표체급림상의의。방법이80례초진방광암환자위연구대상，선택50례방광염환자급50례건강체검자위대조。비교삼조환자뇨액중BLCA-4표체적차이급기여방광암환자림상병리지표적관계。결과방광암환자뇨액중BLCA-4수평현저고우방광염환자급건강체검자，차이구유통계학의의（P＜0．05），방광염급건강체검자지간미견통계학차이（P＞0．05）；BLCA-4대방광암적진단령민도위93．8％，특이도위100．0％；BLCA-4여방광암환자년령、성별、병리분급、종류대소급림상분기등인소균무상관성（ P＞0．05）；복발환자여미복발환자술전급술후6개월뇨액중BLCA-4수평비교균무현저차별（P＞0．05），량조환자술후6개월BLCA-4수평균현저하강（P＜0．05），단술후1년6례복발환자뇨액BLCA-4수평균현저고우술후6개월수평（P＜0．05），차여미복발환자상비차이구유통계학의의（P＜0．05）。결론 BLCA-4대방광암진단민감도고，가강대방광암환자술후BLCA-4수평적감측유조우예측복발정황。
Objective To explore the expression of bladder cancer specific nuclear matrix protein 4(BLCA-4) in urine of bladder cancer patients and its clinical significance.Methods 80 cases of newly diagnosed bladder cancer patients were se-lected as research subjects,and 50 cystitis cases and 50 healthy patients as control subjects.The urine levels of BLCA-4 among the 3 groups were compared and the relationship with clinicopathological indicators were explored.Results The urine level of BLCA-4 in bladder cancer patients was significantly higher than that of cystitis patients and healthy patients(P<0.05);There were no significant difference in BLCA-4 between cystitis patients and healthy patients(P>0.05);The sensitivity of BLCA-4 for bladder cancer was 93.8%and specificity was 100.0%;BLCA-4 had no relationship with age,sex,pathological grading,tumor size and clinical stage( P>0.05);There were no significant difference in BLCA-4 before operation and 6 months after operation between the patients with and without recurrence(P>0.05);The BLCA-4 level greatly decreased 6 months after operation in pa-tients with and without recurrence(P<0.05);1 year after operation,the BLCA-4 level in 6 patients with recurrence was signifi-cantly higher than 6 months after operation, There were significant difference compared with patients without recurrence ( P <0.05).Conclusion BLCA-4 has high sensitivity for bladder cancer.Enhancing monitoring of BLCA-4 is benefical for evaluating recurrence.
