目的:探讨吸入噻托溴铵对稳定期慢性阻塞性肺病(COPD)患者呼吸功能的影响。方法:选取本院呼吸与危重二科就诊的稳定期慢性阻塞性肺疾病患者68例,随机分成吸入噻托溴铵药物组和不吸入对照组,在治疗前、治疗后6周,分别测定并比较治疗前后两组的肺功能(FEV1、FEVl/FVC、FEVl%预计值)和血清炎性介质(IL-8、TNF-α)的浓度。结果:(1)两组治疗前FEV1、FEVl/FVC、FEVl%预计值、IL-8、TNF-α比较,差异均无统计学意义(P>0.05)。(2)治疗第6周末时,两组FEVl、FEVl/FVC、FEVl%预计值均高于治疗前,差异有统计学意义(P<0.01或P<0.05),炎性介质IL-8、TNF-α均低于治疗前(P<0.01),且治疗组比对照组改善更加明显(P<0.01)。结论:吸入噻托溴铵可明显改善慢性阻塞性肺疾病患者的肺功能,降低IL-8、TNF-α水平,减轻气道炎症,延缓肺功能恶化。

목적:탐토흡입새탁추안대은정기만성조새성폐병(COPD)환자호흡공능적영향。방법:선취본원호흡여위중이과취진적은정기만성조새성폐질병환자68례,수궤분성흡입새탁추안약물조화불흡입대조조,재치료전、치료후6주,분별측정병비교치료전후량조적폐공능(FEV1、FEVl/FVC、FEVl%예계치)화혈청염성개질(IL-8、TNF-α)적농도。결과:(1)량조치료전FEV1、FEVl/FVC、FEVl%예계치、IL-8、TNF-α비교,차이균무통계학의의(P>0.05)。(2)치료제6주말시,량조FEVl、FEVl/FVC、FEVl%예계치균고우치료전,차이유통계학의의(P<0.01혹P<0.05),염성개질IL-8、TNF-α균저우치료전(P<0.01),차치료조비대조조개선경가명현(P<0.01)。결론:흡입새탁추안가명현개선만성조새성폐질병환자적폐공능,강저IL-8、TNF-α수평,감경기도염증,연완폐공능악화。
Objective:To investigate the effects of inhaled Tiotropium on pulmonary function results in patients with stable chronic obstructive pulmonary disease(COPD).Method:68 COPD patients were randomly divided into experimental group(tiotropium group) and control group.The pulmonary function(FEVl,FEVl/FVC,FEVl%) and infiammatory cytokines(IL-8,TNF-α) were measured both before and 6 weeks after inhalation.Result:(1)There were no significant differences in the values of forced expiratory volume in FEV1,FEV1/FVC,FEV1%,IL-8 and TNF-α before treatment between two groups (P>0.05).(2)These were significantly improved in the values of forced expiratory volume in FEV1, FEV1/FVC,FEV1%(P<0.01 orP<0.05),the levels of IL-8,TNF-αin induced sputum can be significantly decreased between two groups(P<0.01),and after treatment the experimental group were better than the control group(P<0.01).Conclusion:Inhaled tiotropium can significantly improve the pulmonary functions,reduce the levels of inflammatory cytokines and airway inflammation in the treatment of COPD patients at stable stage.
