目的:对糖化血红蛋白、糖化血清蛋白在妊娠糖尿病早期诊断的临床探讨。方法抽取2012年2月~2014年2月本院接诊的336例妊娠期孕妇作为研究对象,按临床诊断将其分为妊娠糖尿病组(GDM组,n=132例)、糖耐量受损组(IGT组,n=85例)及正常对照组(n=119例),比较三组糖筛查结果、糖化血红蛋白(HbA1c)、糖化血清蛋白(GSP)水平及GDM组、IGT组的HbA1c、GSP异常率。结果 GDM组糖筛查结果、HbA1c、GSP水高于IGT、正常对照组,IGT组糖筛查结果、HbA1c、GSP水平高于正常对照组,三组糖筛查结果、HbA1c、GSP水平差异具有统计学意义(P<0.05);GDM组HbA1c、GSP异常率明显高于IGT组,差异具有统计学意义(P<0.05),GDM组、IGT组组内HbA1c、GSP异常率比较差异具有统计学意义(P<0.05);HbA1c的正确诊断指数稍高于GSP,GSP检测具有较高的灵敏性,HbA1c检测具有较高的特异性及阳性预测值,但差异无统计学意义(P>0.05)。结论糖化血红蛋白、糖化血清蛋白在妊娠糖尿病的早期诊断方面具有重要的应用价值。

목적:대당화혈홍단백、당화혈청단백재임신당뇨병조기진단적림상탐토。방법추취2012년2월~2014년2월본원접진적336례임신기잉부작위연구대상,안림상진단장기분위임신당뇨병조(GDM조,n=132례)、당내량수손조(IGT조,n=85례)급정상대조조(n=119례),비교삼조당사사결과、당화혈홍단백(HbA1c)、당화혈청단백(GSP)수평급GDM조、IGT조적HbA1c、GSP이상솔。결과 GDM조당사사결과、HbA1c、GSP수고우IGT、정상대조조,IGT조당사사결과、HbA1c、GSP수평고우정상대조조,삼조당사사결과、HbA1c、GSP수평차이구유통계학의의(P<0.05);GDM조HbA1c、GSP이상솔명현고우IGT조,차이구유통계학의의(P<0.05),GDM조、IGT조조내HbA1c、GSP이상솔비교차이구유통계학의의(P<0.05);HbA1c적정학진단지수초고우GSP,GSP검측구유교고적령민성,HbA1c검측구유교고적특이성급양성예측치,단차이무통계학의의(P>0.05)。결론당화혈홍단백、당화혈청단백재임신당뇨병적조기진단방면구유중요적응용개치。
Objective To analyze the clinical application of glycated hemoglobin, glycated serum protein in the early diagnosis of gestational diabetes. Methods 336 cases of pregnant women during pregnancy from February 2012 to February 2014 in our hospital were randomly selected as research objects, according to the clinical diagnosis, they were divided into gestational diabetes group (GDM group,n=132), impaired glucose tolerance group (IGT group, n=85) and normal control group (n=119). The glucose screening, HbA1c, GSP levels in serum in three groups and the ab-normal rate of GSP, HbA1c in GDM group, IGT group were compared. Results The glucose screening, HbA1c, GSP levels in serum of GDM were higher than that of IGT and normal control group, the glucose screening, HbA1c, GSP levels in serum of IGT were higher than that of nor-mal control group, the differences of three groups were statistically significant (P<0.05); The HbA1c、GSP abnormal rate of GDM group was higher than that of IGT, the differences were sta-tistically significant (P<0.05), the abnormal rate of HbA1c、GSP in each group by chisquare test had statistical significance(P<0.05); The correct diagnosis index of HbA1c was a little higher than GSP, the GSP test had high sensitivity, HbA1c test had high specificity and positive predictive value, but there was no significant difference (P>0.05). Conclusion Glycosylated haemoglobin and glycosylated serum protein has important clinical application value in the early diagnosis of gesta-tional diabetes.
