目的:比较振动声桥(VSB)与软带BAHA(骨锚式助听器)对双侧先天性中外耳畸形患者听力改善的效果。方法通过2例自身对照患者,比较VSB与软带BAHA使用前后,纯音测听(声场)与安静和噪声下65dBSPL言语识别率。结果2种方式均使患者获得明显的听力改善,在纯音测定(声场)中,气导听阈在1kHz-8kHz,VSB助听后达到正常水平(<25dB),而软带BAHA助听在30-50dB之间,但在0.25kHz与0.5kHz,软带BAHA比VSB能进一步改善10-25dB。安静状态下,对单音节词和双音节词识别率,VSB比BAHA高7%-38%;但安静与噪声环境下句子识别率仅高2-10%。结论VSB与BAHA都是双侧先天性中外耳畸形患者的有效听力解决方案。VSB在高频(1kHz-8kHz)助听效果较BAHA好,但低频(0.25kHz与0.5kHz)反之;安静状态下对单音节词和双音节词识别率VSB更好,而在安静和噪声下对句子识别率VSB比BAHA稍高。

Objective To compare hearing improvement effect of VSB and BAHA (Bone-anchored hearing aids) soft?band on patients with bilateral congenital deformation of middle and outer ears. Method comparing results of pure tone audi?ometry (sound field) and speech recognition rate at 65dB SPL in quiet and noise settings for 2 patients self-control with VSB or BAHA softband. Results The patients obtain obvious hearing improvement in two ways. by the pure tone (sound field) audi?ometry, hearing of air conduction threshold to normal level (<25dB) at1kHz-8kHz with VSB, and 30-50dB with softband BAHA, while at 0.25kHz and 0.5kHz, softband BAHA can obtain further 10-25dB improvement than VSB. In quiet state, speech recognition rates of the monosyllabic words and two syllable words with VSB are higher than those with softband BAHA by 7%-38%;but as for sentence recognition rate, it is only 2-10%difference between VSB and softband BAHA in qui?et and noisy environment. Conclusion VSB and softband BAHA are both effective solution to improve hearing for patients with bilateral congenital deformation of middle and outer ears. At high frequencies (1kHz-8kHz) VSB is better than softband BAHA, however it is vice versa at low frequencies (0.25kHz,0.5kHz). In quiet state speech recognition rates of monosyllabic words and two syllable words with VSB are better than those with softband BAHA, while it is only slight higher to sentences recognition rate in quiet and noisy setting.
