目的：探讨股骨近端抗旋髓内钉不扩髓治疗高龄骨质疏松性股骨转子间骨折的临床效果。方法选取2006年1月~2013年6月在本院治疗的110例高龄骨质疏松性股骨转子间骨折患者作为研究对象，随机分为对照组和观察组，各55例。对照组给予动力髋螺钉治疗，观察组给予股骨近端抗旋髓内钉不扩髓方法治疗，比较两组的手术时间、骨折愈合时间、切口长度、出血量、卧床时间、Barthel生活指数、治疗有效率。结果观察组的手术时间<60min者、骨折愈合时间<8周者显著多于对照组，差异有统计学意义(P<0.05)。观察组的切口长度、卧床时间显著短于对照组，差异有统计学意义(P<0.05)。观察组的出血量显著少于对照组，差异有统计学意义(P<0.05)。观察组的Barthel生活指数、治疗有效率显著高于对照组，差异有统计学意义(P<0.05)。结论股骨近端抗旋髓内钉不扩髓治疗高龄骨质疏松性股骨转子间骨折的临床效果显著，值得临床推广应用。
목적：탐토고골근단항선수내정불확수치료고령골질소송성고골전자간골절적림상효과。방법선취2006년1월~2013년6월재본원치료적110례고령골질소송성고골전자간골절환자작위연구대상，수궤분위대조조화관찰조，각55례。대조조급여동력관라정치료，관찰조급여고골근단항선수내정불확수방법치료，비교량조적수술시간、골절유합시간、절구장도、출혈량、와상시간、Barthel생활지수、치료유효솔。결과관찰조적수술시간<60min자、골절유합시간<8주자현저다우대조조，차이유통계학의의(P<0.05)。관찰조적절구장도、와상시간현저단우대조조，차이유통계학의의(P<0.05)。관찰조적출혈량현저소우대조조，차이유통계학의의(P<0.05)。관찰조적Barthel생활지수、치료유효솔현저고우대조조，차이유통계학의의(P<0.05)。결론고골근단항선수내정불확수치료고령골질소송성고골전자간골절적림상효과현저，치득림상추엄응용。
Objective To explore the clinical effect of unreamed intramedullary proximal femoral nail anti-rotation in the treatment of senile osteoporotic intertrochanteric fractures. Methods 110 cases with senile osteoporotic in-tertrochanteric fracture from January 2006 to June 2013 in our hospital were selected and randomly divided into the control group and the observation group,55 cases in each group.The control group was treated with dynamic hip screw, the observation group was treated with unreamed intramedullary proximal femoral nail anti-rotation.Operation time, fracture healing time,length of incision,amount of bleeding,the time in bed,Barthel index,the treatment efficiency was compared respectively. Results The patients of operation time less than 60min,fracture healing time less than 8 week in the observation group was more than that in the control group,with significant difference(P<0.05).Length of incision,the time in bed of the observation group was shorter than that of the control group,with significant difference (P<0.05).Amount of bleeding of the observation group was less than that of the control group,with significant difference (P<0.05).Barthel in-dex,the treatment efficiency of the observation group was higher than that of the control group,with significant difference (P<0.05). Conclusion The clinical effect of unreamed intramedullary proximal femoral nail anti-rotation in the treat-ment of senile osteoporotic intertrochanteric fractures is significant,it is worthy of clinical promotion and application.
