Dezocine%Ketorolac tromethamine%Postoperative patient-controlled intravenous analgesia
目的:探讨地佐辛复合酮咯酸氨丁三醇在胸科术后静脉自控镇痛(PCIA)中的应用效果。方法选择ASA分级I~Ⅱ级、胸科肺大泡切除术患者40例,随机分为两组,每组20例。I组采用地佐辛30 mg加酮咯酸氨丁三醇150 mg,Ⅱ组采用芬太尼1 mg。两组均用生理盐水稀释至200 ml。手术后接PCIA泵,设置负荷剂量10 ml,持续给药量3 ml/h,自控量4 ml/20 min,记录两组视觉模拟评分(VAS)和Ramsay评分,比较两组镇痛满意度、PCIA泵按压次数和不良反应。结果两组术后VAS、Ramsay及Likert评分比较,差异均无统计学意义(P>0.05)。两组不良反应发生率比较差异有统计学意义(P<0.05)。两组PCIA泵药物用量及使用次数的比较差异均无统计学意义(P>0.05)。结论地佐辛复合酮咯酸氨丁三醇应用于胸科术后PCIA效果确切,安全性高,且不良反应发生少。

목적:탐토지좌신복합동각산안정삼순재흉과술후정맥자공진통(PCIA)중적응용효과。방법선택ASA분급I~Ⅱ급、흉과폐대포절제술환자40례,수궤분위량조,매조20례。I조채용지좌신30 mg가동각산안정삼순150 mg,Ⅱ조채용분태니1 mg。량조균용생리염수희석지200 ml。수술후접PCIA빙,설치부하제량10 ml,지속급약량3 ml/h,자공량4 ml/20 min,기록량조시각모의평분(VAS)화Ramsay평분,비교량조진통만의도、PCIA빙안압차수화불량반응。결과량조술후VAS、Ramsay급Likert평분비교,차이균무통계학의의(P>0.05)。량조불량반응발생솔비교차이유통계학의의(P<0.05)。량조PCIA빙약물용량급사용차수적비교차이균무통계학의의(P>0.05)。결론지좌신복합동각산안정삼순응용우흉과술후PCIA효과학절,안전성고,차불량반응발생소。
Objective To investigate application effect of dezocine combined with ketorolac tromethamine in thoracic postoperative patient-controlled intravenous analgesia. Methods 40 patients in ASAⅠ-II with chest operation resec-tion of lung bullae were selected and randomly divided into two groups,20 patients in each group.Dezocine 30 mg and ketorolac tromethamine 150 mg were used in groupⅠ,while fentanyl 1 mg in groupII.The two groups of drug were di-luted to 200 ml with NS,and the patient-controlled intravenous analgesia pump was set with loading dose 10 ml,contin-uous dosage 3 ml/h,self-control was 4 ml/20 min.The visual analogue score (VAS)and the Ramsay score were recorded, analgesic satisfaction and the pressing frequency of patient-controlled intravenous analgesia pump,adverse reaction were compared. Results The postoperative VAS,Ramsay score and Likert score in two groups was compared respective-ly,with no statistical difference(P>0.05).The incidence rate of adverse reaction in two groups was compared,with statisti-cal difference (P<0.05).The drug dosage and use frequency of patient-controlled intravenous analgesia pump in two groups was compared respectively,with no statistical difference(P>0.05). Conclusion Dezocine combined with ketorolac tromethamine applied in patient-controlled intravenous analgesia after thoracic surgery can provide exact effection,has high safety and less adverse reaction.
