目的:探讨血清同型半胱氨酸(Hcy)与踝肱指数(ABI)在评估糖尿病患者周围动脉病变(PAD)中的相关性。方法选择2011年8月至2012年6月厦门市第二医院收治的2型糖尿病患者204例,均测定血清Hcy及ABI,并根据Hcy 水平分为A 组(Hcy<15μmol/L,167例)和B组(Hcy≥15μmol/L,37例);根据ABI水平分为非 PAD 组(0.9<ABI≤1.3,140例)、PAD组(ABI≤0.9,19例)和动脉硬化组(ABI>1.3,45例),比较不同组间Hcy和ABI水平的变化情况。结果动脉硬化组患者血清Hcy最高,PAD组次之,非PAD组血清Hcy最低,三组两两比较,差异均有统计学意义(P<0.05);B组患者异常ABI的比率明显高于A组,差异有统计学意义(P<0.01)。结论血清Hcy与2型糖尿病PAD关系密切,有望成为糖尿病PAD的血清标志物。

목적:탐토혈청동형반광안산(Hcy)여과굉지수(ABI)재평고당뇨병환자주위동맥병변(PAD)중적상관성。방법선택2011년8월지2012년6월하문시제이의원수치적2형당뇨병환자204례,균측정혈청Hcy급ABI,병근거Hcy 수평분위A 조(Hcy<15μmol/L,167례)화B조(Hcy≥15μmol/L,37례);근거ABI수평분위비 PAD 조(0.9<ABI≤1.3,140례)、PAD조(ABI≤0.9,19례)화동맥경화조(ABI>1.3,45례),비교불동조간Hcy화ABI수평적변화정황。결과동맥경화조환자혈청Hcy최고,PAD조차지,비PAD조혈청Hcy최저,삼조량량비교,차이균유통계학의의(P<0.05);B조환자이상ABI적비솔명현고우A조,차이유통계학의의(P<0.01)。결론혈청Hcy여2형당뇨병PAD관계밀절,유망성위당뇨병PAD적혈청표지물。
Objective To investigate the correlation of homocysteine (Hcy) and ankle brachial index (ABI) in evalua-tion of diabetic peripheral artery disease. Methods ABI and homocysteine were measured in 204 Type 2 diabetic patients from Auguest 2011 to June 2012 in Xiamen No.2 Hospital. According to serum Hcy concentrations ,the patients were divided into group A (Hcy<15μmol/L,167 cases),and group B (Hcy≥15μmol/L,37 cases).According to ABI,the patients were divided into non-PAD(0.9<ABI≤1.30,140 cases),PAD group(ABI≤0.9,19 cases),and the atherosclerosis group(ABI>1.3,45 cases). Results The plasma serum Hcy concentrations in the atherosclerosis group was the highest,followed by PAD group,non-PAD group with the lowest plasma serum Hcy. The were statistical significance in difference (P<0.05). The incidence rate of abnormal ABI of group B were obviously higher that of the group A,whose difference had stastical significance(P<0.01). Conclusion The plasma Hcy is closely related to diabetic peripheral artery disease(PAD),which may be a serum marker for predicting diabetic PAD.
