目的分析313例清洁中段尿培养菌株构成及耐药性分析,为医师合理规范使用抗生素提供依据。方法对医院2012年6月~2013年5月住院泌尿系感染患者尿培养分离出的313株病原菌用法国梅里埃公司VITEK 2 COMPACT全自动微生物鉴定药敏检测系统仪进行鉴定和药敏试验。结果共培养1804份中段尿标本,分离出313例病原微生物,阳性率17.35%。其中革兰阴性杆菌、革兰阳性球菌、真菌、革兰阳性杆菌分别占65.50%、19.81%、7.35%、7.35%。大肠埃希菌仍是泌尿系感染的主要致病菌,其次为肺炎克雷伯杆菌、阴沟肠杆菌、铜绿假单胞菌、奇异变形菌;其中产超广谱β-内酰胺酶(ESBLs)大肠埃希菌占大肠埃希菌41.94%;而在革兰阳性菌中以肠球菌属为主。检出耐亚胺培南的肺炎克雷伯菌1株,耐碳青霉烯类阴沟肠杆菌2株,耐万古霉素屎肠球菌1例。结论泌尿系感染常见病原菌仍为大肠埃希菌,临床应根据尿培养及药敏试验结果有针对性筛选敏感药物进行治疗。

목적분석313례청길중단뇨배양균주구성급내약성분석,위의사합리규범사용항생소제공의거。방법대의원2012년6월~2013년5월주원비뇨계감염환자뇨배양분리출적313주병원균용법국매리애공사VITEK 2 COMPACT전자동미생물감정약민검측계통의진행감정화약민시험。결과공배양1804빈중단뇨표본,분리출313례병원미생물,양성솔17.35%。기중혁란음성간균、혁란양성구균、진균、혁란양성간균분별점65.50%、19.81%、7.35%、7.35%。대장애희균잉시비뇨계감염적주요치병균,기차위폐염극뢰백간균、음구장간균、동록가단포균、기이변형균;기중산초엄보β-내선알매(ESBLs)대장애희균점대장애희균41.94%;이재혁란양성균중이장구균속위주。검출내아알배남적폐염극뢰백균1주,내탄청매희류음구장간균2주,내만고매소시장구균1례。결론비뇨계감염상견병원균잉위대장애희균,림상응근거뇨배양급약민시험결과유침대성사선민감약물진행치료。
Objective To analyze 313 cases of clean urine culture and drug resistance of strains constituting provide evidence for physicians reasonable standard use of antibiotics.Methods Hospital from June 2012 to May 2013 hospitalized patients with urinary tract infection urine culture isolated 313 pathogens with French Merieux VITEK 2 COMPACT automated microbial identification system susceptibility testing instrument for identification and susceptibility testing. Results A total of 1804 copies of urine specimens cultured, pathogenic microorganisms isolated from 313 cases,the positive rate of 17.35%. Gram-negative bacteria,Gram-positive bacteria,fungi,Gram-positive bacil i accounted for 65.50%,19.81%,7.35%,7.35%.Escherichia coli urinary tract infection remains a major pathogen,fol owed by Klebsiel a pneumoniae, Enterobacter cloacae,Pseudomonas aeruginosa,Proteus mirabilis;wherein producing extended spectrumβ-lactamases (ESBLs) Escherichia coli accounted for 41.94 percent;and in Gram-positive bacteria to Enterococcus based. Detection of imipenem-resistant Klebsiel a pneumoniae 1,carbapenem-resistant Enterobacter cloacae 2,vancomycin-resistant Enterococcus one case feces. Conclusion Urinary tract infection remains common pathogens Escherichia coli,should be based on clinical urine culture and sensitivity test results of a targeted screening of sensitive drug.
