目的:检测凋亡抑制基因Bcl-2、上皮钙黏素( E-cadherin)在肾透明细胞癌及癌旁组织中的表达,探讨Bcl-2、E-cadherin在肿瘤发生、发展过程中的作用。方法采用 Elivision 免疫组织化学法对2008年6月至2010年7月广西壮族自治区人民医院泌尿外科68例行根治性手术切除肾透明细胞癌组织的人肾透明细胞癌患者的癌组织及癌组织外缘1.0 cm处组织以及正常肾组织中Bcl-2和E-cadherin基因的表达进行检测,观察Bcl-2、E-cadherin蛋白表达与病理分级间的关系。结果肾透明细胞癌及其癌组织外缘1.0 cm处组织Bcl-2蛋白表达阳性率分别为80.8%和44.1%,显著高于正常肾组织(32.5%),差异有统计学意义(P<0.05);E-cadherin蛋白在肾透明细胞癌和癌组织外缘1.0 cm处癌旁组织表达阳性率分别为13.2%和91.7%,显著低于正常肾组织(100%),差异有统计学意义(P <0.05)。Bcl-2在肾透明细胞癌高分化中的阳性表达率最低,E-cadherin在肾透明细胞癌高分化中阳性表达率最高,差异有统计学意义(P<0.05)。结论 Bcl-2、E-cadherin参与了肾透明细胞癌的发生和发展,Bcl-2、E-cadherin可能成为肾透明细胞癌潜在的基因治疗靶点和病情进展的预测指标。

Objective To detect the expression of Bcl-2 and E-cadherin in renal clear cell carcinoma and pericarcinous tissue,and discuss their role in the development of tumor .Methods 68 cases with renal clear cell carcinoma in the People's Hospital of Guangxi Zhuang Autonomous Region were detected from Jun .2008 to Jul.2012.The expression of Bcl-2 and E-cadherin in cancer tissue,1.0 cm from the outer edge of cancer tis-sue,and normal nephridial tissue were observed by immunohistochemical SP method ,and their relationship with pathological grade .Results The positive rate of Bcl-2 in renal clear cell carcinoma and 1.0cm from pericarci-nous tissue were 80.8%and 44.1%,respectively,obviously higher than 32.5%in normal nephridial tissue(P<0.05).The positive rate of E-cadherin in renal clear cell carcinoma and 1.0cm from the outer edge of cancer tissue were 13.2%and 91.7%,obviously lower than 100%in normal nephridial tissue(P<0.05).the positive expression rate of Bcl-2 in renal clear cell carcinoma was the lowest than that in other tissues ,positive expression rate of E-cadherin in renal clear cell carcinoma was higher than that in tissues ,the difference was statistically significant(P<0.05).Conclusion Bcl-2 and E-cadherin play an important role in the develop-ment of renal clear cell carcinoma .They could be the possible target for gene therapy and predictive index for the development of disease.
