目的:探讨同型半胱氨酸(Hcy)与原发性高血压患者肾功能关系。方法纳入2012年我院原发性高血压患者共814例。据Hcy值分为H型高血压(Hcy≥10μmol/L)与非H型高血压(Hcy<10μmol/L)组,分别比较两组患者的肾功能的差异,采用线性回归模型分析Hcy与肾功能相关指标间的关系。结果 H型高血压患者肌酐[(105.9±47.1)μmol/L vs.(88.6±55.9)μmol/L,P<0.001]、尿酸[(360.2±106.2)mmol/L vs.(309.4±83.8)mmol/L,P<0.001]显著高于非H型高血压组,eGFR[(57.1±5.0)ml·min-1·(1.73 m2)-1vs.(68.8±23.4)ml·min-1·(1.73 m2)-1,P<0.001]显著低于非H型高血压组。回归分析发现高血压患者中Hcy水平与肌酐(β=1.542,P<0.001)、尿酸(β=2.845,P<0.001)呈正相关,与eGFR呈负相关(β=-0.726,P<0.001)。H型高血压患者中Hcy水平与肌酐(β=1.028,P<0.001)、尿酸(β=1.661,P<0.001)呈正相关,与eGFR呈负相关(β=-0.482,P<0.001)。结论 Hcy与原发性高血压患者肾功能相关,尤其是H型高血压患者。

목적:탐토동형반광안산(Hcy)여원발성고혈압환자신공능관계。방법납입2012년아원원발성고혈압환자공814례。거Hcy치분위H형고혈압(Hcy≥10μmol/L)여비H형고혈압(Hcy<10μmol/L)조,분별비교량조환자적신공능적차이,채용선성회귀모형분석Hcy여신공능상관지표간적관계。결과 H형고혈압환자기항[(105.9±47.1)μmol/L vs.(88.6±55.9)μmol/L,P<0.001]、뇨산[(360.2±106.2)mmol/L vs.(309.4±83.8)mmol/L,P<0.001]현저고우비H형고혈압조,eGFR[(57.1±5.0)ml·min-1·(1.73 m2)-1vs.(68.8±23.4)ml·min-1·(1.73 m2)-1,P<0.001]현저저우비H형고혈압조。회귀분석발현고혈압환자중Hcy수평여기항(β=1.542,P<0.001)、뇨산(β=2.845,P<0.001)정정상관,여eGFR정부상관(β=-0.726,P<0.001)。H형고혈압환자중Hcy수평여기항(β=1.028,P<0.001)、뇨산(β=1.661,P<0.001)정정상관,여eGFR정부상관(β=-0.482,P<0.001)。결론 Hcy여원발성고혈압환자신공능상관,우기시H형고혈압환자。
ObjectiveTo explore the association of homocysteine(Hcy) with renal function patients in primary hypertension.Methods814 cases with primary hypertension were included in 2012. They were divided into H-type hypertension group (Hcy≥10μmol/L) and non H-type hypertension group (Hcy<10μmol/L) according to the Hcy level. The differences in renal function between groups were compared. The linear regression model was used to analyze the relationship between Hcy and parameters of renal function.ResultsThe mean age of H-type hypertension and non H-type hypertension were 71.4±11.9 and 67.0±11.2 years. Creatinine (105.9±47.1)μmol/Lvs. (88.6±55.9)μmol/L (P<0.001), uric acid (360.2±106.2)mmol/Lvs. (309.4±83.8)mmol/L (P<0.001), were higher in H-type hypertension, while eGFR (57.1±5.0)ml·min-1·(1.73 m2)-1vs. (68.8±23.4)ml·min-1·(1.73 m2)-1(P<0.001), was lower. The regression analysis found that Hcy level was positively related with creatinine (β=1.542,P<0.001) and uric acid (β=2.845,P<0.001), while negatively related with eGFR (β=-0.726,P<0.001) in primary hypertension patients. Furthermore, Hcy level was also positively related with creatinine (β=1.028,P<0.001) and uric acid (β=1.661,P<0.001), while negatively related with eGFR (β=-0.482,P<0.001) in H-type hypertension patients.ConclusionHcy level is associated with renal function in primary hypertension patients, especially in H-type hypertension patients.
