目的：探讨肾上腺皮质癌的临床病理学特点及诊断要点。方法对5例肾上腺皮质癌进行临床资料、病理形态学及免疫组化观察，并结合文献对其诊断及鉴别诊断进行探讨。结果镜下可见瘤细胞呈多边形，细胞质丰富，嗜酸性，核染色质呈粗颗粒状；瘤细胞侵及包膜；瘤巨细胞，核仁明显，粗颗粒状，胞浆嗜酸性；瘤细胞呈巢状，片状生长。免疫组化结果肿瘤细胞Vimentin和α-inhibin表达阳性。结论提高对肾上腺皮质癌的认识，对避免误诊是至关重要的。
목적：탐토신상선피질암적림상병이학특점급진단요점。방법대5례신상선피질암진행림상자료、병리형태학급면역조화관찰，병결합문헌대기진단급감별진단진행탐토。결과경하가견류세포정다변형，세포질봉부，기산성，핵염색질정조과립상；류세포침급포막；류거세포，핵인명현，조과립상，포장기산성；류세포정소상，편상생장。면역조화결과종류세포Vimentin화α-inhibin표체양성。결론제고대신상선피질암적인식，대피면오진시지관중요적。
Objective To explore the clinicopathologic characteristics and diagnostic criteria of adrenal cortical carcinoma. Methods Clinical data, pathomorphology and immunohistochemical staining of 5 patient with adrenal corti-cal carcinoma were observed, and its diagnosis and differential diagnosis combined with literature were also analyzed. Re-sults Microscopic examination showed that the tumor cells were polygonal, cytoplasm was rich and eosinophilic, and nu-clear chromatin was granular coarse;tumor cells invaded into the envelope;giant-cell tumors and prominent nucleoli were found with coarse granular and eosinophilic cytoplasm;tumor cells were nidulant and flaky growth. Immunohistochemical staining showed that tumor cells positively expressed Vimentin and α-inhibin. Conclusion It is essential to enhance awareness of pathological characteristics of adrenal cortical carcinoma to avoid misdiagnosis.
