intrapericardial treatment of inferior pulmonary vein%video-assisted thoracic surgery (VATS)%resection of partial left atrium%lobectomy
Objective: To investigate the indication, technique, safety and effectiveness when intrapericardial treatment of inferior pulmonary vein and resection of partial left atrium are applied in complete thoracoscopic lobectomy. Methods: A total of 28 patients, including 16 males and 12 females, who had an average age of 57.2(range,44-71) and were treated with intrapericardial separation of inferior pulmonary vein and resection of partial left atrium from February, 2011 to December, 2013, were reviewed. The patients had a diameter of inferior pulmonary tumor greater than 5cm(range,5-8.2cm) which might involve adjacent pericardium, or had difficulty in separating inferior pulmonary vein outside the pericardium. Results: Al 28 patients had been treated with intrapericardial separation of inferior pulmonary vein smoothly. Thoracoscope was combined with straight-cutting stitching instrument to separate inferior pulmonary vein and remove partial left atrium. During surgery, no blood vessel ruptured and no massive haemorrhage occurred, 5 cases (17.9%) have transient cardiac arrhythmia, No patients died during perioperative period. Conclusions: The intrapericardial treatment of inferior pulmonary vein and resection of partial left atrium can expand the range of application of complete thoracoscopic lobectomy, and the treatment of blood vessel inside pericardium can be regarded as a safe and reliable way.
