目的:研究旨在探讨超高液相色谱(Ultrahigh pressure liquid chromatography, UPLC)法检测的精准性及5-甲基四氢叶酸(5-methyltetrahydrofolate,5-MTHF)测定方法学的建立。方法用UPLC法检测不同间隔时间及浓度MTX、CF及5-MTHF样品,分析UPLC法检测的精准性。结果 UPLC法检测3种浓度(1.0 mg/L 4.0 mg/L 16.0 mg/L)的血浆样品室温放置48h后测定的浓度与标示量的相对偏差分别为MTX 5.6%、5%、2.5%;CF 2%、5%、5.6%;5-MCHF 6.0%、7.5%、5.6%;-80摄氏度冻融1月后测定的浓度与标示量的相对偏差分别为MTX 5.6%、5%、2.5%;CF1.2%、2.5%、1.3%;5-MCHF5.4%、2.5%、6.3%。结论本法测定儿童血浆中5-MTHF的浓度时结果稳定,重现性好,可以用于临床大剂量MTX化疗后CF解救的患儿体内血浆中解救药物CF浓度监测,增加了MTX应用的进一步监测指标,为以后探讨5-MTHF浓度高时ALL患儿的复发风险提供了有力的检测手段。

Objective The study was to explore ultra high performance liquid chromatography (Ultrahigh pressure liquid chromatography, UPLC) method to detect precision and folinic acid (5 - methyl (5 -methyltetrahydrofolate, 5 - MTHF) determination of the establishment of the methodology. Methods Using UPLC method to detect different interval time and the concentration of MTX, CF and samples of 5 -MTHF analysis UPLC method detection precision. Results The UPLC method to detect three kinds of concentration (1.0 mg/L, 4.0 mg/L, 16.0 mg/L) of plasma samples at room temperature for 48 h after the determination of the concentration and the relative deviation of the amount of MTX 5.6%, 5%and 5.6%respectively;CF were 2%, 5%, 5.6%; 5 - MCHF 6.0%, 7.5%, 5.6%; - 80 degrees Celsius freeze-thaw in January after the determination of the concentration and the relative deviation of the amount of MTX were 5.6%, 5% and 5.6%respectively; CF were 1.2%, 2.5%, 1.3%; 5 - MCHF 5.4%, 2.5% and 6.3%.Conclusion The determination of 5-MTHF in plasma of children this method when the results are stable, provides a powerful means for the detection of recurrence risk after the discussion of the higher concentration of 5-MTHF in children with ALL.
