目的:研究帕金森病(PD)患者双侧丘脑和双侧基底节区磁共振扩散张量成像(DTI)的变化,评价DTI 技术用于早期PD 的诊断价值。方法:以PD 患者26例为PD 组,按Hoehn-Yahr 分级量表将患者分为PD 组1和PD 组2;健康志愿者26例为对照组;常规行 MR 头部平扫和 DTI 检查,比较双侧丘脑、基底节区的部分各向异常分数(FA)值和表观弥散系数(ADC)值。结果:PD 组尾状核和丘脑的部分各向异常分数 FA 值明显低于对照组(P<0.05),PD 组兴趣区的ADC 值在各组之间、病变双侧之间和对照组之间无明显差异(P>0.05),但随着病情级别的增加,ADC 值呈增高趋势。结论:磁共振DTI 的尾状核、丘脑的FA 值有助于PD 的早期诊断。

목적:연구파금삼병(PD)환자쌍측구뇌화쌍측기저절구자공진확산장량성상(DTI)적변화,평개DTI 기술용우조기PD 적진단개치。방법:이PD 환자26례위PD 조,안Hoehn-Yahr 분급량표장환자분위PD 조1화PD 조2;건강지원자26례위대조조;상규행 MR 두부평소화 DTI 검사,비교쌍측구뇌、기저절구적부분각향이상분수(FA)치화표관미산계수(ADC)치。결과:PD 조미상핵화구뇌적부분각향이상분수 FA 치명현저우대조조(P<0.05),PD 조흥취구적ADC 치재각조지간、병변쌍측지간화대조조지간무명현차이(P>0.05),단수착병정급별적증가,ADC 치정증고추세。결론:자공진DTI 적미상핵、구뇌적FA 치유조우PD 적조기진단。
Objective: To study the change of bilateral thalamus and bilateral basal ganglia region diffusion tensor imaging (DTI) of patients with Parkinson's disease (PD) to evaluate the early diagnosis value of DTI technology for PD. Methods: 26 patients with PD as group PD, and divided into PD group 1 and PD group 2 according to the Hoehn-Yahr grading scales, while 26 healthy volunteers as control group. All objects were conventional MR scan head and DTI checks. The fractional anisotropy (FA)and apparent diffusion coefficient (ADC)values of bilateral thalamus and basal ganglia region were compared between the two groups. Results: FA values of the caudate nucleus and thalamus in PD group was significantly lower than that in the control group (P< 0.05). ADC values of interest areas in PD group in both PD group 1 and 2 , the both pathological sides and the control group were no significant difference (P>0.05), but with the increase of illness level, an increased trend of ADC values. Conclusion:It is help for the early diagnosis of PD on caudate nucleus and thalamus FA values of magnetic resonance DTI.
