目的:采用序贯法测定异丙酚(丙泊酚)抑制小剂量氯胺酮兴奋循环作用的半数有效剂量(ED50)。方法择期全身麻醉患者21例,按照序贯法给药原则:丙泊酚剂量按等比数列逐级增加,公比为1.2,初始剂量为0.5 mg/kg。根据静脉注射氯胺酮后血压、脉搏变化情况调整丙泊酚的剂量。记录各时间点的血压、脉搏和SpO2数值及镇痛镇静评分(OAA/S),使用加权均数法计算ED50。结果异丙酚抑制小剂量氯胺酮兴奋循环作用的ED50为0.56 mg/kg,95%可信区间为(0.54,0.62)。结论异丙酚可以有效抑制小剂量氯胺酮的交感神经兴奋作用,两药配伍使用血压、脉搏平稳,呼吸抑制轻微。

목적:채용서관법측정이병분(병박분)억제소제량록알동흥강순배작용적반수유효제량(ED50)。방법택기전신마취환자21례,안조서관법급약원칙:병박분제량안등비수렬축급증가,공비위1.2,초시제량위0.5 mg/kg。근거정맥주사록알동후혈압、맥박변화정황조정병박분적제량。기록각시간점적혈압、맥박화SpO2수치급진통진정평분(OAA/S),사용가권균수법계산ED50。결과이병분억제소제량록알동흥강순배작용적ED50위0.56 mg/kg,95%가신구간위(0.54,0.62)。결론이병분가이유효억제소제량록알동적교감신경흥강작용,량약배오사용혈압、맥박평은,호흡억제경미。
Objective To determine the median effective dose (ED50) of propofol in inhibition of mall dose ketamine excited circulation by sequential method. Methods There were 21 cases of general anesthesia. Sequential method medication principle was propofol dose increasing by geometric progression with the common ratio as 1.2, and the initial dose was 0.5 mg/kg. Propofol dose was adjusted by changes of blood pressure and pulse after intravenous injection of ketamine. Records were made at each time point for blood pressure, pulse, SpO2, and observer assessment of sedation (OAA/S). ED50 was calculated by weighted mean method. Results The ED50 of propofol in inhibition of mall dose ketamine excited circulation was 0.56 mg/kg, and 95%confidence interval was (0.54, 0.62). Conclusion Propofol can effectively inhibit the sympathetic excitatory effect of small-dose ketamine. Combined application of the two drugs can make blood pressure and pulse stable with mild respiratory depression.
