目的:观察Narcotrend监测对七氟醚-瑞芬太尼麻醉妇科腹腔镜手术患者的应用效果,评价其对患者苏醒期的影响。方法60例妇科腹腔镜手术患者,随机分为Narcotrend监测组(A组)和临床组(B组),记录手术时间、瑞芬太尼用量、患者自主呼吸恢复时间、呼之睁眼时间、拔管时间、定向力恢复时间及Narcotrend 指数(NTI)、观察患者不良反应发生情况,比较两组数据有无差异。结果 A组手术时间和瑞芬太尼用量明显减少,差异具有统计学意义(P<0.05);与B组比较, A组患者的自主呼吸恢复时间、呼之睁眼时间、拔管时间、定向力恢复时间显著缩短,差异具有统计学意义(P<0.05);A组患者苏醒拔管及定向力恢复NTI数值比呼之睁眼的NTI数值显著增加,差异具有统计学意义(P<0.05)。A组不良反应少于B组,差异均有统计学意义(P<0.05)。结论 Narcotrend监测能够减少七氟醚-瑞芬太尼麻醉剂量,有助于缩短患者苏醒时间,减少不良反应的发生,提高患者麻醉质量。

목적:관찰Narcotrend감측대칠불미-서분태니마취부과복강경수술환자적응용효과,평개기대환자소성기적영향。방법60례부과복강경수술환자,수궤분위Narcotrend감측조(A조)화림상조(B조),기록수술시간、서분태니용량、환자자주호흡회복시간、호지정안시간、발관시간、정향력회복시간급Narcotrend 지수(NTI)、관찰환자불량반응발생정황,비교량조수거유무차이。결과 A조수술시간화서분태니용량명현감소,차이구유통계학의의(P<0.05);여B조비교, A조환자적자주호흡회복시간、호지정안시간、발관시간、정향력회복시간현저축단,차이구유통계학의의(P<0.05);A조환자소성발관급정향력회복NTI수치비호지정안적NTI수치현저증가,차이구유통계학의의(P<0.05)。A조불량반응소우B조,차이균유통계학의의(P<0.05)。결론 Narcotrend감측능구감소칠불미-서분태니마취제량,유조우축단환자소성시간,감소불량반응적발생,제고환자마취질량。
Objective To observe the application effect of Narcotrend monitoring for patients undergoing gynecological laparoscopic operation with sevoflurane-remifentanil anesthesia, and to evaluate its influence on recovery period of patients. Methods A total of 60 patients undergoing gynecological laparoscopic operation were randomly divided into Narcotrend monitoring group (group A) and clinical group (group B). Records were made on operation time, dose of remifentanil, recovery time of autonomous respiration, called eye opening time, extubation time, orientation recovery time, and Narcotrend index (NTI). Adverse reactions of patients were observed, and data of the two groups were compared to show their differences. Results Group A had lowered operation time and dose of remifentanil, and the difference had statistical significance (P<0.05). Compared with group B, group A had obviously shorter recovery time of autonomous respiration, called eye opening time, extubation time, and orientation recovery time. The difference had statistical significance (P<0.05). The group A had more increased NTI in recovery extubation and orientation than in called eye opening, and the difference was statistically significant (P<0.05). The group A had fewer adverse reactions than group B, and the difference had statistical significance (P<0.05). Conclusion Application of Narcotrend monitoring can reduce the dose of sevoflurane-remifentanil anesthesia, and it is also helpful for shorten recovery time and reduce adverse reactions. It can improve anesthesia quality for patients.
