目的：比较宫颈扩张球囊及地诺前列酮栓(欣普贝生)用于妊娠期糖尿病(GDM)引产的有效性和安全性。方法150例足月GDM初产妇采用随机数字表法分成治疗组与对照组，各75例，治疗组患者进行COOK宫颈扩张球囊促宫颈成熟治疗，对照组患者进行阴道后穹窿放置欣普贝生促宫颈成熟治疗。对两种方法的促宫颈成熟效果、临产时间、分娩方式、血糖变化及不良反应进行分析比较。结果治疗组促宫颈成熟的效果及阴道分娩率与对照组相近(P>0.05)，不良反应低于对照组，差异有统计学意义(P<0.05)。结论对足月GDM初产妇采用COOK宫颈扩张球囊及欣普贝生均可以有效促进宫颈成熟， COOK宫颈扩张球囊的不良反应少，值得推广。
목적：비교궁경확장구낭급지낙전렬동전(흔보패생)용우임신기당뇨병(GDM)인산적유효성화안전성。방법150례족월GDM초산부채용수궤수자표법분성치료조여대조조，각75례，치료조환자진행COOK궁경확장구낭촉궁경성숙치료，대조조환자진행음도후궁륭방치흔보패생촉궁경성숙치료。대량충방법적촉궁경성숙효과、임산시간、분면방식、혈당변화급불량반응진행분석비교。결과치료조촉궁경성숙적효과급음도분면솔여대조조상근(P>0.05)，불량반응저우대조조，차이유통계학의의(P<0.05)。결론대족월GDM초산부채용COOK궁경확장구낭급흔보패생균가이유효촉진궁경성숙， COOK궁경확장구낭적불량반응소，치득추엄。
Objective To compare the effectiveness and safety between cervical dilation balloon and dinoprostone suppositories (Propess) applied for labor induction in gestational diabetes mellitus (GDM). Methods A total of 150 full-term primipara with GDM were divided into treatment group and control group by random number table, and each group contained 75 cases. The treatment group received COOK cervical dilation balloon for promoting cervical ripening treatment, and the control group received Propess in posterior fornix for promoting cervical ripening treatment. Analysis and comparisons were made between effects of promoting cervical ripening, labor time, delivery ways, blood glucose changes and adverse reactions of the two groups. Results The treatment group had similar effects of promoting cervical ripening and vaginal delivery rate as the control group (P>0.05). Its adverse reaction was lower than the control group, and the difference had statistical significance (P<0.05). Conclusion COOK cervical dilation balloon and Propess both can promote cervical ripening in full-term primipara with GDM. COOK cervical dilation balloon has few adverse reactions, and it is worthy of promotion.
