目的:应用经胸彩色多普勒超声对冠状动脉狭窄的血流动力学进行研究,了解其诊断冠状动脉狭窄的价值。方法85例将进行冠状动脉造影检查的患者,按照造影结果提示冠状动脉狭窄程度>70%和<70%分成狭窄组52例和对照组33例。在造影检查前应用经胸彩色多普勒超声检测冠状动脉血流速度,对出现局部血流加速者,计算加速处与加速前血流速度比值。结果狭窄组中38例可测及局部加速血流,其血流速度明显高于加速前及对照组血流速度,加速处与加速前血流速度比值>2.0,可作为诊断冠状动脉狭窄的指标。结论经胸彩色多普勒超声能够检测冠状动脉狭窄的血流信号,通过测量相关血流动力学参数,可以为判断冠状动脉狭窄程度提供有价值的信息。

Objective To apply transthoracic color Doppler ultrasound to research haemodynamics in coronary stenosis, and to uinderstand its value in diagnosis of coronary stenosis. Methods A total of 85 patients for coronary arteriography were divided into stenosis group with 52 cases and control group with 33 cases, according to coronary stenosis degree>70%and<70%by results of arteriography. Transthoracic color Doppler ultrasound was applied during arteriography to detect blood flow velocity in coronary artery. The blood flow velocity ratio of accelerated position and before acceleration was calculated in accelerate blood flow patients. Results There were 38 cases in the stenosis group with detected local accelerate blood flow, and their blood flow velocity was higher than that before acceleration and the control group. The ratio of accelerated position and before acceleration>2.0, and that could be used as the indicator for diagnosing coronary stenosis. Conclusion Transthoracic color Doppler ultrasound can detect blood flow signal in coronary artery. By detecting related hemodynamic parameters, it can provide valuable information for coronary stenosis degree.
