目的：对比关节置换法和内固定法治疗老年骨质疏松合并股骨粗隆间骨折的临床疗效及其骨密度的影响。方法选取老年骨质疏松合并股骨粗隆间骨折患者60例作为研究对象，随机分为两组。实验组采取对关节置换法治疗，对照组采取内固定法治疗。观察两组治疗后临床疗效、并发症发生率和骨密度。结果两组患者住院时间、手术时间、出血量、下地半年后 Harris 评分比较无明显差异（ P ﹥0.05）；实验组的下地时间明显小于对照组，两组比较差异具有统计学意义（ P ﹤0.05）。实验组并发症总发生率明显低于对照组，两组比较差异具有统计学意义（ P ﹤0.05）。两组治疗前骨密度值无明显差异（ P ﹥0.05），实验组治疗后骨密度值优于对照组，差异具有统计学意义（ P ﹤0.05）。结论关节置换法治疗老年骨质疏松合并股骨粗隆间骨折临床疗效满意，可早期下地，降低并发症的发生率，增强骨密度，优于内固定治疗法。
목적：대비관절치환법화내고정법치료노년골질소송합병고골조륭간골절적림상료효급기골밀도적영향。방법선취노년골질소송합병고골조륭간골절환자60례작위연구대상，수궤분위량조。실험조채취대관절치환법치료，대조조채취내고정법치료。관찰량조치료후림상료효、병발증발생솔화골밀도。결과량조환자주원시간、수술시간、출혈량、하지반년후 Harris 평분비교무명현차이（ P ﹥0.05）；실험조적하지시간명현소우대조조，량조비교차이구유통계학의의（ P ﹤0.05）。실험조병발증총발생솔명현저우대조조，량조비교차이구유통계학의의（ P ﹤0.05）。량조치료전골밀도치무명현차이（ P ﹥0.05），실험조치료후골밀도치우우대조조，차이구유통계학의의（ P ﹤0.05）。결론관절치환법치료노년골질소송합병고골조륭간골절림상료효만의，가조기하지，강저병발증적발생솔，증강골밀도，우우내고정치요법。
Objective To contrast the clinical effect of joint replacement fixed method and internal fixation on treating osteoporosis com-plicated with intertrochanter fracture of femur. Methods To choose 60 cases with osteoporosis complicated with intertrochanter fracture of femur who was accepted by our hospital as the research subject. They were equally and randomly divided into control group and experiment group. Pa-tients in the experiment group were treated with joint replacement immobilization osteoporosis while the patients in the control group were treated with internal fixation. Results The contrasting the length of hospital stay,the operating time,the amount of bleeding,the Harris score after up and about six months in two groups have no significant difference( P ﹤ 0. 05). The overall complication rate in the experiment group was obviously lower than that in the control group. The significance betwwen two groups was significant( P ﹤ 0. 05). There was no significant difference in BMD of two groups before treating( P ﹥ 0. 05). BMD in the experimental group after treatment was better than in the control group,the difference was statistically significant( P ﹤ 0. 05). Conclusion Treatment of joint replacement method osteoporotic intertrochanteric fracture consolidation have satisfactory clinical efficacy,early ambulation,can reduce the incidence of complications and increase bone density.
