目的：评价肝移植术后使用免疫抑制剂他克莫司（Tac）和环孢素（CsA）的疗效差异和安全性。方法采用中英文数据库进行文献检索，英文数据库包括 Pubmed、Medline、Embase 和 Cochrane library，中文数据库包括CNkI、VIP、万方。纳入随机对照试验（RCT），对纳入的文献进行方法学评价和 Meta 分析。结果共纳入17篇 RCTs；Meta 分析结果显示，Tac 对比 CsA，患者生存率差异无统计学意义，移植物存活率和急性排斥反应 Tac 优于 CsA，但他克莫司组的糖尿病发生率较高，差异具有统计学意义。结论肝移植术后 Tac 抗排斥反应治疗效果优于 CsA，但会导致更多患者糖尿病的发生。
목적：평개간이식술후사용면역억제제타극막사（Tac）화배포소（CsA）적료효차이화안전성。방법채용중영문수거고진행문헌검색，영문수거고포괄 Pubmed、Medline、Embase 화 Cochrane library，중문수거고포괄CNkI、VIP、만방。납입수궤대조시험（RCT），대납입적문헌진행방법학평개화 Meta 분석。결과공납입17편 RCTs；Meta 분석결과현시，Tac 대비 CsA，환자생존솔차이무통계학의의，이식물존활솔화급성배척반응 Tac 우우 CsA，단타극막사조적당뇨병발생솔교고，차이구유통계학의의。결론간이식술후 Tac 항배척반응치료효과우우 CsA，단회도치경다환자당뇨병적발생。
Objective To compare the therapeutic effect difference and safety of tacrolimus and cyclosporine as pri-mary immunosuppressant for liver transplantation recipients. Methods Searches were applied to the Database in English and Chinese:English database includes:Pubmed、Medline、Embase and Cochrane library;Chinese database includes:CNkI、VIP、WANGFANG;Randomized controlled trials(RCT)comparing Tac with CsA after liver transplantation were included. The meth-odological quality was assessed and Meta analysis,which were performed with Ravman5. 2. Results A total of 17 RCTs were included,The results of Meta-analysis of indicated the patientsˊsurvival rate was not significantly different between tacrolimus and cyclosporine,graft survival rate and the incidence of acute rejection in Tac group is superior to CsA,but the incidence rate of diabetes in Tac group was significantly higher. Conclusion Compared to cyclosporine,tacrolimus significantly reduced the risk after liver transplantation of acute rejection,but tacrolimus increased the risk of new _ onset diabetes.
