基于RE-AIM模型（可及性，效果，支持，项目实施，持续性），在分解和归纳基本公共卫生服务项目活动基础上，建立基本公共卫生服务项目实施障碍因素分析框架，并以广州市社区卫生服务机构为例，对基本公共卫生服务项目实施障碍因素进行定量描述，以期为确定项目未来改进方向与重点，推动项目持续发展和科学化进程提供政策建议。

This paper expounded the establishment of a framework to expose the disorder factors through the implementation process of the basic public health services by applying the RE-AIM framework(reach,effectiveness, adoption,implementation,and maintenance),combining with the basis of resolving and concluding the project activities. As a case study of Guangzhou community health service institutions,the implementation disorder factors of basic public health services were quantitatively described. It is expected that the case study could make some policy advices about the future improvement direction of the project and the promotion of the sustainable development of the project and the scientific process.
