目的:研究促炎因子肿瘤坏死因子(TNF-α)在脓毒症患儿标准化治疗过程中的变化情况。方法设立对照分析,脓毒症患儿50例为实验组进行标准化治疗,测定入院第1天、第4天和第7天以及出院或死亡当天外周血TNF-α含量和简化危重评分PCIS;对照组选择健康体检儿童10例,测定外周血TNF-α含量。结果对照组TNF-α含量均在正常范围内;实验组TNF-α在第1天、第4天、第7天和死亡当天时间的含量逐渐降低,组内两两比较差异有统计学意义(P<0.05);实验组死亡1例,为感染性休克合并急性肾功能衰竭患者;实验组平均住院治疗时间为13.5 d。结论 TNF-α为脓毒症患儿促炎因子之一,可作为诊断脓毒症的辅助指标,同时也可作为判断脓毒症疗效的手段。

Objective To study the proinflammatory cytokine tumor necrosis factor (TNF-α) therapy in children with sepsis in the standardization process changes.Methods The establishment of comparative analysis, 50 cases of children with sepsis standardized treatment for the experimental group, measured one day admission, the first four days and day 7 and the day of discharge or death peripheral blood TNF-α content and simplify critical score PCIS; selecting healthy children in the control group 10 cases, the content of TNF-α was measured in peripheral blood.Results The control group TNF-α levels were within the normal range; TNF-α in the experimental group, one day, four day, day 7 content and Tx time decreased, the group was statistically significant pairwise comparison(P<0.05); the experimental group died one case, for septic shock in patients with acute renal failure; the experimental group, the average hospitalization time was 13.5 days.Conclusion TNF-α is one of pro-inflammatory cytokines in children with sepsis, sepsis can be used as a diagnostic aid targets, but also could be used as a means of treatment of sepsis.
