目的：探讨弹性髓内钉治疗儿童股骨干骨折远期疗效的影响因素，评价弹性髓内钉治疗儿童股骨干骨折的临床应用价值。方法收集2011年2月—2013年9月在我院诊治且病历资料完整，采用闭合复位钛制弹性髓内钉内固定术治疗的儿童股骨干骨折患者71例，观察其远期疗效的影响因素。结果71例患儿优42例、良14例、可15例、无效0例，优良率为78.87%。多因素Logistic回归分析表明：年龄、体重、骨折类型、复位质量、髋关节及膝关节功能锻炼情况是影响弹性髓内钉治疗儿童股骨干骨折远期疗效的主要因素。结论弹性髓内钉治疗儿童股骨干骨折远期疗效较好；年龄11岁~13岁、骨折类型为其他型、复位质量残存移位>3 mm、超正常体重>20%、患肢未进行髋膝关节功能锻炼等，是影响其远期疗效的主要因素。

ObjectiveTo explore long-term clinical efficacy's influence factors of using elastic intramedullary nail treat children's femoral shaft fractures,and evaluate the clinical application value of elastic intramedullary nail treat Children's femoral shaft fractures.MethodsCollected 71 children patients from No.1 Department of Orthopaedics and Traumatology,Chinese Medicine Hospital,Qiannan,Guizhou Province from February 2011 to September 2013,their medical records were complete and treated by closed reset titanium elastic intramedullary nail fixation technology,used forward stepwise method to do unconditioned logistic multifactor analysis.Results71 children include 42 excellent patients,14 good patients and 15 effective patients,the excellent and good rate is 78.87%. Multifactor Logistic regression analysis shows that,the main factors of impacting long-term clinical efficacy of using elastic intramedullary nail treat children's femoral shaft fractures are:age,weight,type of fracture,reset quality,hip and knee joints' exercise situations.ConclusionThe long-term efficacy of using elastic intramedullary nail treat children's femoral shaft fractures is good;the main factors of impacting long-term clinical efficacy of using elastic intramedullary nail treat children's femoral shaft fractures are:aged 11-13 years old,with other fracture types,reset quality residual displacement>3mm,exceed normal weight of >20%,the affected limb fails to carry out hip and knee joints'exercise,etc.
