目的:探讨不同剂量右旋美托咪定的镇静效果及其对机体呼吸循环系统的影响。方法选择30名支气管镜检查患者并随机分为3组,每组患者选择不同剂量Dex(0.5、1.0及1.5μg/kg)静脉注射,观察Dex的临床效应及这种效应是否为浓度依赖性。结果注射后即产生明显的镇静效应,表现为警觉/镇静评分明显降低(P<0.05),并伴随心率下降(P<0.05),15 min后受试者出现血氧饱和度降低(P<0.05)。同时亦可降低三组受试者的血压(P<0.05),但三组起效的起始时间不同。剂量因素、时间和剂量的交互因素差异无统计学意义(P>0.05)。结论静脉注射0.5-1.5 ug/kg Dex,出现明显的镇静效应,且在15 min内完成检查最佳,同时Dex能有效降低受试者的血压、血氧饱和度及心率,这种效应非药物浓度依赖性,但随着药物量的增加,其恢复时间随之延长。

목적:탐토불동제량우선미탁미정적진정효과급기대궤체호흡순배계통적영향。방법선택30명지기관경검사환자병수궤분위3조,매조환자선택불동제량Dex(0.5、1.0급1.5μg/kg)정맥주사,관찰Dex적림상효응급저충효응시부위농도의뢰성。결과주사후즉산생명현적진정효응,표현위경각/진정평분명현강저(P<0.05),병반수심솔하강(P<0.05),15 min후수시자출현혈양포화도강저(P<0.05)。동시역가강저삼조수시자적혈압(P<0.05),단삼조기효적기시시간불동。제량인소、시간화제량적교호인소차이무통계학의의(P>0.05)。결론정맥주사0.5-1.5 ug/kg Dex,출현명현적진정효응,차재15 min내완성검사최가,동시Dex능유효강저수시자적혈압、혈양포화도급심솔,저충효응비약물농도의뢰성,단수착약물량적증가,기회복시간수지연장。
Objective To explore the sedative effect of different doses of dexmedetomidine and its effect on respiratory and circulatory system. Methods 30 bronchoscopy patients were randomly divided into three groups to be given a different dose of dexmedetomidine intravenously (0. 5, 1. 0 and 1. 5 μg/kg). Their clinical effect was ob-served. Results Dexmedetomidine had a significant inhibitory effect, which the score of OAA/S was significantly lower and the heart rate also decreased (P<0. 05). The subjects appeared the reduction of blood oxygen saturation 15 minutes after dexmedetomidine injection ( P<0. 05 ) , and dexmedetomidine also showed antihypertensive effect (P<0. 05), but the initial effective time was different. There was no significant difference in does-effect and dose-time interaction (P>0. 05). Conclusion All the patients have sedative effect when they are injected 0. 5 -1. 5μg/kg dexmedetomidine. This check could be better in 15 minutes. At the same time, dexmedetomidine could effec-tively reduce blood pressure, oxygen saturation and heart rate.
