目的:探讨几种不同方法检测痰中抗酸杆菌的临床应用价值。方法病例组385例和对照组80例痰标本均采用直接涂片法、液基夹层杯离心集菌涂片(夹层杯法)、噬菌体生物扩增法( PhaB法)、BacT/Alert 3D全自动系统分离培养法(简称3D培养法)检测,对结果比较分析。结果对照组中夹层杯法和PhaB法各有一例假阳性;病例组中3D培养法检出率56.1%为最高,但耗时长,费用高,操作繁琐;PhaB法检出率53.2%,快速且可作药敏试验;夹层杯法检出率40.0%,操作简便、安全、快速;直接涂片法检出率20.5%为最低,但价廉。以3D培养法结果为标准,直接涂片法、夹层杯法、PhaB 法的灵敏度、特异度、准确度分别为33.8%、95.9%、61.0%;66.7%、94.1%、78.7%;89.8%、94.0%、91.4%。结论夹层杯法和PhaB法可作为直接涂片法和培养法的重要补充或是互补的方法,既可提高检出率又可作药敏试验,且快速及时为临床提供诊断依据和用药指导,可在基层医院推广。

목적:탐토궤충불동방법검측담중항산간균적림상응용개치。방법병례조385례화대조조80례담표본균채용직접도편법、액기협층배리심집균도편(협층배법)、서균체생물확증법( PhaB법)、BacT/Alert 3D전자동계통분리배양법(간칭3D배양법)검측,대결과비교분석。결과대조조중협층배법화PhaB법각유일례가양성;병례조중3D배양법검출솔56.1%위최고,단모시장,비용고,조작번쇄;PhaB법검출솔53.2%,쾌속차가작약민시험;협층배법검출솔40.0%,조작간편、안전、쾌속;직접도편법검출솔20.5%위최저,단개렴。이3D배양법결과위표준,직접도편법、협층배법、PhaB 법적령민도、특이도、준학도분별위33.8%、95.9%、61.0%;66.7%、94.1%、78.7%;89.8%、94.0%、91.4%。결론협층배법화PhaB법가작위직접도편법화배양법적중요보충혹시호보적방법,기가제고검출솔우가작약민시험,차쾌속급시위림상제공진단의거화용약지도,가재기층의원추엄。
Objective To investigate the clinical application value of different detection methods of acid fast bacilli in sputum. Methods It selected 385 patients as the study group and 80 patients as the control group, and all of them were given direct smear method, sandwich cup method, phage biological amplification ( PhaB) , and BacT/Alert 3D automatic system isolation method (3D culture method) detection. The results were comparatively analyzed. Results In the control group, sandwich cup method and PhaB method had one false positive case. In the study group, the detection rate of 3D culture method was the highest (56. 1%), but it cost longer time and higher expense. The detection rate of PhaB was 53. 2%, with the advantages of easy operation and drug sensitive test. The detection rate of sandwich cup method was 40%, which was simple, safe, quick and convenient. The detection rate of direct smear method was 20. 5%, which was the lowest but cheap. Taking the results of 3D culture method results as the standard, the sensitivity, specificity and accuracy were 33. 8%, 95. 9%, 61%; 66. 7%, 94. 1%, 78. 7%; and 89. 8%, 94%, 91. 4% respectively by direct smear method, sandwich cup method, and PhaB method. Conclusion Sandwich cup method and PhaB method can be used as an important supplement of direct smear method and culture method or complementary approach, which can improve the detection rate and can be used for drug sensitive test.
