目的:探讨神经生长因子(NGF)联合自体骨髓干细胞(BMSCs)动员治疗重型颅脑损伤的临床疗效和安全性。方法选取60例重型颅脑损伤患者按随机数字表分为对照组(30例,给予N G F和综合康复治疗)和观察组(30例,在对照组治疗基础上加用自体BMSCs动员治疗),比较2组患者神经功能缺损评分(NIHSS)、格拉斯哥预后评分(GOS)及不良反应。结果2组患者治疗28 d的NIHSS评分均较治疗前明显降低(P<0.05),且观察组较对照组降低更为显著(P<0.05);观察组伤后6个月GOS评分明显高于对照组(P<0.05),外周血CD34+、CD133+占外周血单核细胞比率治疗3周时明显高于对照组(P<0.05)。2组患者均未出现明显不良反应。结论 NGF联合自体BMSCs动员及综合康复治疗可促进重型颅脑损伤患者神经损伤的修复,显著改善患者神经功能。

Objective To investigate the curative effects of NGF combined with autologous bone marrow stem cell mobili‐zation (BMSCs) on treating patients with severe craniocerebral injury.Methods 60 patients with severe craniocerebral injury were randomly divided into control group (n=30, treated with NGF plus comprehensive rehabilitation therapy) and observation group (n=30, treated with NGF, comprehensive rehabilitation therapy plus BMSCs).The neurological function deficit, prog‐nosis and adverse reaction of two groups were evaluated by the neurological function deficit score (NIHSS) and Glasgow Out‐come Scale (GOS).Results The NIHSS of two groups at 28 d after treatment was lowered than that before treatment, espe‐cially in the observation group;the GOS of observation group was higher than that of control group at 6 months after treat‐ment ;the peripheral blood CD34+ and CD133+ ratio of peripheral blood mononuclear cells of observation group at 3 weeks af‐ter treatment was higher than that of control group.There was no obvious adverse reaction in two groups.Conclusion The NGF, comprehensive rehabilitation therapy plus BMSCs can effectively improve the nervous injury?repair and nervous function of patients w ith severe craniocerebral injury.
