目的：探讨影响老年糖尿病痛性神经病变患者治疗效果的因素，并提出相应的干预措施。方法选取老年糖尿病痛性神经病变患者581例，随机分为2组，对所有患者的临床资料进行回顾分析。结果治疗无效患者中年龄≥70岁、FBG＞6.1 mmol／L、HbA1c≥8.2％、单纯口服药物治疗以及治疗前疼痛程度为轻中度者明显多于治疗显效或有效的患者（P＜0.05）。经过Logistic回归分析，年龄≥70岁、FBG＞6.1 mmol／L、HbA1c≥8.2％、单纯口服药物治疗是影响老年糖尿病痛性神经病变的独立危险因素（ P＜0.05）。结论影响老年糖尿病痛性神经病变治疗效果的因素包括治疗方法的选择、年龄、血糖控制情况等，临床工作中应给予高龄患者更多关注，同时积极控制血糖水平，选择适宜治疗方法，给予必要的心理辅导，指导患者正确认识治疗效果。

Objective To investigate the factors in elderly patients with painful diabetic neuropathy and propose appropri‐ate interventions. Methods Totally 581 cases of elderly patients with painful diabetic nerve disease were randomly divided into two groups. The clinical data of all patients were analyzed retrospectively. Results Non‐responding patients with aged≥70 years ,FBG>6.1 mmol/L ,HbA1c≥8.2% ,simple oral medication for the treatment and mild or moderate pain before treat‐ment were more than the patients with effective efficacy (P< 0.05). Logistic regression analysis indicated that the age≥70 years ,FBG>6.1mmol/L ,HbA1c≥8.2% ,oral medication alone were independent risk factors for the elderly diabetic painful neuropathy (P<0.05). Conclusion The factors affecting the elderly diabetic neuropathy pain ,including the choice of treat‐ment ,the patients’ age ,blood glucose controlling ,etc. Therefore ,more attention should be given to elderly patients ,while actively controlling blood sugar levels in patients and selecting the appropriate method of treatment ,giving patients the necessa‐ry psychological counseling ,guiding patients to a correct understanding of the therapeutic effect.
