目的：对 X 线片（DR）、常规 CT（CCT）及高分辨率 CT（HRCT）对矽肺患者的诊断价值进行比较。方法对收集到的110例明确诊断为矽肺的患者分别采用 CR、CCT 及 HRCT 检查，对三种影像学检查结果进行对比分析。结果三种检测方法对矽肺患者的发白区、大结节影、钙化、灶周气肿、灶周结节、纤维索条影、淋巴结肿大钙化的检出率比较差异显著，其中 DR 对发白区的检出率显著高于 CCT 和 HRCT（ P <0.05）；CCT 和 HRCT 对大结节影、钙化、灶周气肿、灶周结节、纤维索条影、淋巴结肿大钙化的检出率显著高于 DR（ P <0.05）。三种检出方法读片结果的分期比较差异显著（ P <0.05），DR 对Ⅲ期＋Ⅲ＋期矽肺患者的检出不敏感，CCT 和 HRCT 对Ⅲ期＋Ⅲ＋期检出率高于 DR。结论对于矽肺的检查 CT 和 HRCT 具有更高的价值，但结合 DR 检查具有更好的效果。

목적：대 X 선편（DR）、상규 CT（CCT）급고분변솔 CT（HRCT）대석폐환자적진단개치진행비교。방법대수집도적110례명학진단위석폐적환자분별채용 CR、CCT 급 HRCT 검사，대삼충영상학검사결과진행대비분석。결과삼충검측방법대석폐환자적발백구、대결절영、개화、조주기종、조주결절、섬유색조영、림파결종대개화적검출솔비교차이현저，기중 DR 대발백구적검출솔현저고우 CCT 화 HRCT（ P <0.05）；CCT 화 HRCT 대대결절영、개화、조주기종、조주결절、섬유색조영、림파결종대개화적검출솔현저고우 DR（ P <0.05）。삼충검출방법독편결과적분기비교차이현저（ P <0.05），DR 대Ⅲ기＋Ⅲ＋기석폐환자적검출불민감，CCT 화 HRCT 대Ⅲ기＋Ⅲ＋기검출솔고우 DR。결론대우석폐적검사 CT 화 HRCT 구유경고적개치，단결합 DR 검사구유경호적효과。
Objective To investigate the value of X - ray(CR),conventional CT(CCT)and high resolution CT(HRCT),and com-pare the value of diagnosis in patients with silicosis. Methods 110 patients in our hospital was collected for definite diagnosis of silicosis were treated by CR,CCT and HRCT examination. The examination result on the three imaging results were compared and analyzed. Results Three methods for the detection of patients with silicosis white areas,large nodules,calcification,peri - focal emphysema,peripheral nodules,fibrous streaks,enlargement of lymph node calcification detection rate are significantly different. The detection rate of X - ray on the white area was signif-icantly higher than that of CCT and HRCT( P < 0. 05). CCT and HRCT on the nodules,calcification,perifocal emphysema,peripheral nodules, fibrous streaks,enlargement of lymph node calcification detection rate was significantly higher than that of X - ray( P < 0. 05). There was signifi-cant difference in the three kinds of detection methods in the staging( P < 0. 05). X - ray is not sensitive to the detection stage Ⅲ + III + sili-cosis patients,CCT and HRCT in stage Ⅲ + III + phase detection rate is higher than that of X - ray film. Conclusion CT and HRCT has a higher value for the examination of silicosis,but the combination of X - ray examination is better.
