目的:研究分析本院外科手术后病人铜绿假单胞菌的分布及对常用抗生素的耐药情况,以指导临床合理使用抗菌药物。方法收集本院外科手术后病人分离的铜绿假单胞菌,利用WalkAway-96plus全自动微生物鉴定及药敏分析系统和NC50鉴定板进行细菌鉴定及药敏试验,对临床常用15种抗菌药物的耐药性进行分析。结果外科手术后病人铜绿假单胞菌主要分离于痰液中,以呼吸道感染为主,对阿米卡星、头孢噻肟、头孢曲松、庆大霉素、左旋氧氟沙星5种药物的耐药率均大于或等于50%,对头孢他啶、哌拉西林/他唑巴坦、哌拉西林、舒普深耐药率均低于20%。铜绿假单胞菌存在多重耐药株、泛耐药株,在本研究中各占41.3%和1.7%。结论外科病区铜绿假单胞菌的耐药现象比较严重,出现多重耐药株或泛耐药株,应引起临床高度重视。

목적:연구분석본원외과수술후병인동록가단포균적분포급대상용항생소적내약정황,이지도림상합리사용항균약물。방법수집본원외과수술후병인분리적동록가단포균,이용WalkAway-96plus전자동미생물감정급약민분석계통화NC50감정판진행세균감정급약민시험,대림상상용15충항균약물적내약성진행분석。결과외과수술후병인동록가단포균주요분리우담액중,이호흡도감염위주,대아미잡성、두포새우、두포곡송、경대매소、좌선양불사성5충약물적내약솔균대우혹등우50%,대두포타정、고랍서림/타서파탄、고랍서림、서보심내약솔균저우20%。동록가단포균존재다중내약주、범내약주,재본연구중각점41.3%화1.7%。결론외과병구동록가단포균적내약현상비교엄중,출현다중내약주혹범내약주,응인기림상고도중시。
Objective To analyze the distribution of Pseudomonas aeruginosa in patients after surgery and drug resistance to frequently-used antibiotics in our hospital and to guide the clinical rational use of antimicrobial agents.Methods Pseudomonas aeruginosa isolates were collected from the patients after surgery in our hospital. The identiifcations of bacteria and drug sensitivity tests were made using the identiifcation and drug susceptibility analysis system of WalkAway-96plus automatic microorganism and NC50 identification plate. The drug resistance of 15 kinds of frequently-used antibiotics was analyzed.Results Pseudomonas aeruginosa in the patients after surgery in our hospital are mainly isolated from sputum in the patients with respiratory tract infection.Resistance rates to Amikacin,Cefotaxime, Ceftriaxone,Levofloxacin,Gentamicin were greater than or equal to 50% and were lower than 20% to Ceftazidime, Piperacillin/Tazobactam, Piperacillin, Sulperazone. Pseudomonas aeruginosa has multiple resistant strains, pan drug resistant strains, each accounted for 41.3% and 1.7%respectively in this paper.Conclusion The drug resistance of Pseudomonas aeruginosa in the department of surgery is more serious, there are multi-resistant strains and pan resistant strains, which should arouse more attention for surgeon in clinic.
