Laparoscopic%Combined endoscopic%Laparotomy%Cholecystolithiasis combined with extrahepatic bile duct stone
目的：对腹腔镜联合内镜与开腹手术治疗胆囊结石合并肝外胆管结石的疗效进行对比。方法：2011年1月-2014年6月收治胆囊结石合并肝外胆管结石患者84例，分为对照组与观察组，对照组使用开腹手术治疗，而观察组使用腹腔镜联合内镜微创手术治疗，对两组患者的术中出血量、住院时间、手术时间等进行对比与分析。结果：观察组患者的术中出血量、住院时间、术后排便时间、术后进食时间分别是(59.41±4.3)mL、(5.3±1.3)d、(1.7±0.4)d、(2.1±0.2)d，对照组分别是(81.79±9.8)mL、(10.3±2.7)d、(3.2±0.7)d、(3.7±0.6)d，存在较大差异，具有统计学意义(P＜0.05)。结论：腹腔镜联合内镜在胆囊结石合并肝外胆管结石治疗中的应用，创伤小、出血少、住院时间短、痛苦轻，应用前景较好。

목적：대복강경연합내경여개복수술치료담낭결석합병간외담관결석적료효진행대비。방법：2011년1월-2014년6월수치담낭결석합병간외담관결석환자84례，분위대조조여관찰조，대조조사용개복수술치료，이관찰조사용복강경연합내경미창수술치료，대량조환자적술중출혈량、주원시간、수술시간등진행대비여분석。결과：관찰조환자적술중출혈량、주원시간、술후배편시간、술후진식시간분별시(59.41±4.3)mL、(5.3±1.3)d、(1.7±0.4)d、(2.1±0.2)d，대조조분별시(81.79±9.8)mL、(10.3±2.7)d、(3.2±0.7)d、(3.7±0.6)d，존재교대차이，구유통계학의의(P＜0.05)。결론：복강경연합내경재담낭결석합병간외담관결석치료중적응용，창상소、출혈소、주원시간단、통고경，응용전경교호。
Objective:To compare the curative effects of laparoscopic combined endoscopic and laparotomy in the treatment of cholecystolithiasis combined with extrahepatic bile duct stone.Methods:84 cholecystolithiasis patients with extrahepatic bile duct stone were selected from January 2011 to June 2014.They were divided into the control group and the observation group.The control group were treated with laparotomy.The observation group were treated with laparoscopic combined transanal endoscopic microsurgery.The intraoperatve blood soss,hospitalization time and operation time of patients in two groups were compared and analyzed.Results:The intraoperatve blood soss,hospitalization time,postoperative defecation time and postoperative eating time of patients in the observation group were (59.41±4.3)mL,(5.3±1.3)days,(1.7±0.4)days,(2.1±0.2)days.Those of the control group were (81.79±9.8)mL,(10.3±2.7)days,(3.2±0.7)days,(3.7±0.6)days.There was difference with statistical significance(P<0.05). Conclusion:The application of laparoscopic combined endoscopic and laparotomy in the treatment of cholecystolithiasis combined with extrahepatic bile duct stone has small trauma,less bleeding,shorter hospitalization time,less pain.Its application prospect is better.
