目的:探讨不同分期糖尿病视网膜病变(DR)患者血清脂蛋白(α)[LP(a)]和D-二聚体(D-D)水平变化及意义。方法:将126例2型糖尿病患者根据眼底检查及眼底荧光造影结果分为3组:糖尿病无视网膜病变期(NDR)、单纯型糖尿病视网膜病变期(SDR)和增殖型糖尿病视网膜病变期(PDR)。另选40例健康者为正常对照(NC)组。测定各组患者及健康者血清 LP(α)和 D-D 水平,同时研究 LP(α)和 D-D 水平的相关性。结果:LP(α)水平:NC 组<NDR 组<SDR 组<PDR 组,各组间比较差异均有统计学意义(P<0.05)。D-D 水平:NC 组<NDR 组<SDR 组<PDR 组,各组间比较差异均有统计学意义(P<0.05)。血清LP(α)和D-D的相关性:直线相关分析表明,血清LP(α)和D-D呈正相关(r=0.502,P<0.01)。结论:联合检测血清 LP(α)和 D-D 水平有助于早期诊断 DR,并判断其严重程度,为临床上通过降低血清LP(α)、D-D水平控制和延缓糖尿病视网膜病变的发展提供依据。

목적:탐토불동분기당뇨병시망막병변(DR)환자혈청지단백(α)[LP(a)]화D-이취체(D-D)수평변화급의의。방법:장126례2형당뇨병환자근거안저검사급안저형광조영결과분위3조:당뇨병무시망막병변기(NDR)、단순형당뇨병시망막병변기(SDR)화증식형당뇨병시망막병변기(PDR)。령선40례건강자위정상대조(NC)조。측정각조환자급건강자혈청 LP(α)화 D-D 수평,동시연구 LP(α)화 D-D 수평적상관성。결과:LP(α)수평:NC 조<NDR 조<SDR 조<PDR 조,각조간비교차이균유통계학의의(P<0.05)。D-D 수평:NC 조<NDR 조<SDR 조<PDR 조,각조간비교차이균유통계학의의(P<0.05)。혈청LP(α)화D-D적상관성:직선상관분석표명,혈청LP(α)화D-D정정상관(r=0.502,P<0.01)。결론:연합검측혈청 LP(α)화 D-D 수평유조우조기진단 DR,병판단기엄중정도,위림상상통과강저혈청LP(α)、D-D수평공제화연완당뇨병시망막병변적발전제공의거。
Objective:To explore the changes and significances of serum lipoprotein(a) [LP( α)] and D-dimer(D-D) levels of patients with diabetic retinopathy(DR) in different stages.Methods:According to fundus examination and fundus fluorescence angiography results,126 patients with type 2 diabetes were divided into 3 groups:diabetes without retinopathy period(NDR),simple diabetic retinopathy period(SDR) and proliferative type diabetic retinopathy period(PDR).40 healthy persons were selected as the normal control group(NC).The serum LP(α) and D-D levels of patients and healthy persons were detected,and the correlation between the LP(α) and D-D levels were studied.Results:LP(α) levels:NC group<NDR group<SDR group<PDR group,there were statistically significant differences between groups were compared(P<0.05).D-D levels:NC group<NDR group<SDR group<PDR group,there were statistically significant differences between groups were compared(P<0.05).The correlation between serum LP(α) and D-D:linear correlation analysis showed that serum LP(α) and D-D were positively correlated(r=0.502, P<0.01).Conclusion:Combined detection of serum LP(α) and D-D level is helpful to the early diagnosis of DR,and determine its severity.It provides the basis for lowering the serum LP( α) and D-D level to control and delay the development of diabetic retinopathy in clinical.
