目的:通过查找血清特异性IgE,总结荨麻疹可能引起的病因。方法抽取患者血清,采用全自动免疫印迹仪加用吸入性及食入性过敏源特异性IgE抗体检测试剂盒(欧蒙印迹法)来测定患者血清中特异性IgE的浓度,判断荨麻疹的病因。结果吸入性特异性IgE检出率依次为艾蒿70.3%,蟑螂35.6%,屋尘13.4%,葎草11.1%,尘螨组合(屋尘螨/粉尘螨)11.1%,树组合(柳树/杨树/榆树)9.5%,点青霉/分支孢霉/烟曲霉/交链孢霉9.5%,普通豚草8.3%,猫毛7.1%,狗上皮2.7%;食入性特异性IgE检出率依次为海洋鱼组合(鳕鱼/龙虾/扇贝)63.2%,蟹25.6%,花生28.1%,淡水组合(鲑鱼/鲈鱼/鲤鱼)14.2%,蛋清11.1%,牛奶11.1%,牛肉11.1%,羊肉11.1%,虾11.1%,黄豆8.3%。单种致敏原58例,占22.9%,多种致敏原(两种及两种以上)165例,占65.2%,未测出致敏原的30例,占11.8%。结论引起荨麻疹的病因复杂,吸入物和食入物皆有,且可能同时存在,吸入物以艾蒿为最多,食入物以海鲜类最多,且同一患者致敏原常为多种。

목적:통과사조혈청특이성IgE,총결담마진가능인기적병인。방법추취환자혈청,채용전자동면역인적의가용흡입성급식입성과민원특이성IgE항체검측시제합(구몽인적법)래측정환자혈청중특이성IgE적농도,판단담마진적병인。결과흡입성특이성IgE검출솔의차위애호70.3%,장랑35.6%,옥진13.4%,률초11.1%,진만조합(옥진만/분진만)11.1%,수조합(류수/양수/유수)9.5%,점청매/분지포매/연곡매/교련포매9.5%,보통돈초8.3%,묘모7.1%,구상피2.7%;식입성특이성IgE검출솔의차위해양어조합(설어/룡하/선패)63.2%,해25.6%,화생28.1%,담수조합(해어/로어/리어)14.2%,단청11.1%,우내11.1%,우육11.1%,양육11.1%,하11.1%,황두8.3%。단충치민원58례,점22.9%,다충치민원(량충급량충이상)165례,점65.2%,미측출치민원적30례,점11.8%。결론인기담마진적병인복잡,흡입물화식입물개유,차가능동시존재,흡입물이애호위최다,식입물이해선류최다,차동일환자치민원상위다충。
Objective This article is aiming to find out the possible causes of urticaria by analysing 253 patients’serum SIgE. Methods Serum and red cell were separated and serum were used to allergen screening test to find out the allergen by testing specific IgE concentration with food SIgE antibody and inhaled SIgE antibody test reagents. In inhaled group allergens are mugwort 70.3%, cockroach 35.6%, dust 13.4%, scandent hop11.1%, dust mite 11.1%, trees 9.5%, penicillin 9.5%, ragweed 8.3%, cat hair 7.1%, dog fur 2.7%..In ingested group, the positive rates of different allergens are shellfish 63.2%, crab 25.6%, peanuts 28.1%, freshwater fish 14.2%, egg white 11.1%, milk 11.1%, beef 11.1%, lamb 11.1%, shrimp 11.1%, soya beans 8.3%. 22.9%of patients only had one aller?gen which are 58 cases. 65.2%of patients had two or more allergens which are 165 cases. 11.8%of patients which are 30 cases did not have any allergen detected. Conclusion There are no definitive causes for urtarcia. The most common inhaled allergen is mugwort and the most common food allergen is seafood. Most of patients with urtarcia have two or more allergens.
